The morning routine for all four of us was slightly off kilter. My daughter, who usually doesn’t have the energy to complain about starting her day, suggested that she really needed a day off. Sorry, but that wasn’t going to happen. Besides, she doesn’t generally want a day at home because she feels as if she would fall behind in her classes and would rather keep pace.

I dropped her off at school as she groaned something to the effect of, “Bye, have a good day, I hate this.”

I returned to pick up my wife and take her to the train. She was also slightly behind schedule. My son came “elephanting” down the stairs. It’s an expression we use that is exactly as it sounds. He throws his feet so heavily and loudly on the steps that the house shakes until he reaches the first floor, turns hard to his right twice and collapses into his chair.

My wife and I raced out of the house two minutes behind our usual departure time. Two minutes? How was I supposed to make it to a train that is only early when we’re late? It’s Murphy’s law of trains. Whatever can go wrong with the commute does go wrong and, often, in conjunction with other problems.

We came to the final light seconds before the train was scheduled to pull in.

We reached the traffic light just as it turned red, in that small window when all the lights are red at the same time. Despite the line to my left waiting for a green light, I made a right on red and pulled into the intersection behind another car waiting to make the immediate left into the train station.

Unfortunately, the cars on the other side of the street hadn’t left an opening for the frantic commuters to reach the station. When their light changed, the traffic immediately started moving, blocking us from making the turn.

My wife considered getting out, racing across the street and trot-running through the parking lot. The cars speeding by near her door made that impossible.

A car behind me honked, moved to our right and slowly passed. A woman in her 60s flipped us the bird.

Do we still do that? Do we still raise our middle finger to strangers? I do it to my computer when it’s frozen and to my phone when it’s not allowing me to respond to an email or text, or when it adds an error to one of my emails because it retyped a name into something potentially problematic.

But this woman, with her tight lips, curled and dyed hair, and menacing eyes, slowly rolled past me, extending the curse finger just in front of her left shoulder. That raised digit was so stiff, long and rigid that it looked it could have just as easily have been a weapon as a gesture.

I was stunned to react immediately. Then a few responses ricocheted around my head as my wife raced out of the car: “Sorry? Right back at you, sweetheart.” … “You know what you can do with that finger.”

It’s possible her day had, or was expected to have, much bigger problems than mine. I am sorry I upset her so much that she needed to express her outrage.

Or maybe I gave her a chance to be angry at something other than herself, her family, her boss or the people who work for her. Could I have done her a favor, providing a target for her anger?

I don’t know her story, but I do know that my day suddenly seemed less problematic.

A recurring battle along the North Shore that we’re noticing is the struggle communities go through to maintain historical characteristics while also satisfying modern business needs.

Where town or village codes may be lacking to maintain historical and/or architectural cohesion, community leaders are recognizing the importance of creating visioning plans. Our hope is that the want for sense of place is mixed with the needs of businesses in order to fill empty storefronts when crafting each plan in order to create a healthy mix.

Setting up guidelines to maintain its architectural heritage and cohesion is something Port Jefferson Village is paying attention to. At the end of last year, a draft resolution based on a meeting of the village’s architectural review committee was introduced. If passed, it would require new buildings in the village’s commercial districts to adhere to designs consistent with Port Jeff’s “Victorian, maritime heritage” and to avoid a “hodgepodge” of buildings. The policy is far from complete but standards are being discussed, and that’s a good start.

Constructing a visioning plan, with the assistance of residents and business owners, would be beneficial for revitalization in areas like Broadway in Rocky Point. Setauket and Stony Brook residents took a step in the right direction when community leaders, residents and business owners met in 2016 and 2017 to create the Route 25A Three Village Area Visioning Report. The report, approved by the Brookhaven Town Board and pending the adoption of a land-use study by the town’s planning department, creates guidelines for issues that affect the Three Village area including maintaining cohesive architecture.

It gave the Three Village Civic Association some backup when it opposed the owners of a Shell gas station in Setauket on Route 25A applying for variances to the town’s Board of Zoning Appeals. The company submitted proposed plans to construct a large canopy and a lighted electric sign at the gas station. The board closed an April 18 hearing without a decision and, according to town guidelines, has 62 days to make one. While the owners say most gas stations have canopies, residents at the hearing provided evidence to the contrary along Route 25A between St. James and Port Jefferson.

If the gas station doesn’t get its way with its plans, we doubt it will vacate the premises. But what about other cases when a business owner feels an addition would attract more customers? This is when a visioning plan created with history in mind, but also present business needs can have the most impact. During discussions, compromise may be the key.

Northport Village has been able to strike such an agreement. Last summer, the village board was approached about building a hotel at 225 Main St. — something unheard of before then. While residents criticized the proposed plans, the village approved a code modification to make way for the inn. Then the village’s architectural review board toured the 1950s building to determine firsthand if it had any historic value, before allowing the proposed plans to move forward. This two-step process allowed for a democratic proceeding, while protests may have otherwise left empty storefronts or rundown properties standing as eyesores, which is not the best option.

With some discussion, civic-minded folks with a respect for historical aspects can keep business districts from looking like an unattractive mixture of buildings. Taking in the concerns of business owners, can keep those buildings filled.

It actually makes me cringe when I hear discussions questioning whether a college education is worth the expense. Yes, college loans carried by students after they graduate are astronomical and unprecedented. The average student loan debt for the Class of 2016, for example, is $37,172, up 6 percent from the preceding year. Americans owe, in total, more than $1.48 trillion in student loans spread out over 44 million borrowers, more than the $620 billion owed on credit cards, according to figures obtained from the Student Loan Hero website. Average monthly student loan repayment after graduation, for borrowers 20 to 30 years of age, is $351.

Those are, of course, mammoth numbers that are hard to conceive. But how about this for comparison: Mortgage debt is $8.8 trillion. You can move out of a house, but you only have one head. And what you fill that head with can determine the quality of the rest of your life. Your house may contain your financial equity, but your knowledge base and critical thinking make up your life’s equity.

I know the stories about the college dropouts who become billionaires. Good for them, they don’t have to worry about money. But that is part of the point I am trying to make. Education is not only about money, about the job you will hold or the amount of toys you will own by the time you die. Education is partially about income, as statistics prove. College grads earn more in the course of their lives than high school grads. And while today’s auto mechanic, who goes to a vocational school and who is really a kind of computer engineer can earn as much, perhaps, as a doctor or lawyer, money is not the only value in life. Satisfaction, a key ingredient of happiness, is another.

So what do you get from a college education? Is it worth the price?

First let’s talk about price. In the United States, where education is viewed as the ladder to success, a traditional college education at a fine college has always been ranked at the top of the pyramid. Those schools are also the most expensive because they are mainly private. There are various scholarships to help, but for most without adequate resources those schools can be out of reach. Then there are public universities, many of which are exemplary and much cheaper, particularly if you live in state. And three cheers for the two-year community colleges that can carry you halfway to a college degree with truly minimum expense. There are also work-study schools that may take longer to graduate from, but who is holding a stopwatch on your life?

Anyway, what you get out of college is directly proportional to what you put in. Like the computer expression, it’s garbage in, garbage out. So what is the bottom line here? What can you expect to get out of a good, traditional college?

For starters, there is knowledge, knowledge about almost everything known to humans at the time you
attend. It’s there for the asking, assuming there is room for you to enroll in the classes of your choice. And if you go on to college reasonably soon after you graduate from high school, you can focus on acquiring the knowledge of your choice without the responsibilities of a spouse, a car, a house, children, a dog and making a living. In college, you have a roof over your head, your meals are prepared and the lawn is mowed for you. The knowledge you choose to acquire may or may not turn out to be directly applicable to the work that you eventually do, but it will certainly contribute to your understanding of your world — scientifically, culturally, historically, economically, politically, and that will give you profound satisfaction. If your job depends on what you know rather than how much you can lift, knowledge will extend your work life, at the senior end when those whose bodies can no longer respond to physical tasks may face uncertain “golden yea
rs.”

Learning, of course, doesn’t depend on or stop with a college education. But appreciation for the value of knowledge grows as we age. Boy, how I wish I could live again those college years. Now I would know why I was there.

Fresh Pea Soup

If you’re thinking of not bothering to read this because you have better things to do than shell peas, relax. This article is about those emerald green ones that are an early spring crop and that you buy, if you happen to be  into shelling peas, in the produce department. Otherwise, you buy them frozen, and they’re almost as good, certainly good enough for the recipes that follow. If you think peas are half of the vegetable called peasandcarrots that you grew up with, think again. This is about fresh pea soup with mint, a lovely spring dish served hot or warm and a refreshing summer dish, served cold. It’s about pasta with peas, asparagus and arugula and, yes, for nostalgia’s sake, it’s even about peasandcarrots but in a tangy rice salad.

Fresh Pea Soup

Fresh Pea Soup

YIELD: Makes 6 servings

INGREDIENTS: 

2 tablespoons unsalted butter

2 cups chopped Vidalia onion

1 cup chopped fresh fennel

4 to 5 cups chicken broth

Two 10-ounce packages frozen peas or 5 cups shelled fresh peas

½ cup chopped fresh flat-leaf parsley

2 teaspoons fresh tarragon leaves

Salt and freshly ground pepper, to taste

½ cup heavy cream

DIRECTIONS: 

In a large saucepan, melt the butter. Add the onion and fennel and, stirring frequently, cook over medium-low heat until they begin to soften, about 5 to 7 minutes. Add broth, peas, parsley, tarragon, salt and pepper; then cook 10 minutes over medium heat until vegetables are tender but peas are still bright green. Reserve two tablespoons peas. 

Let soup cool 10 minutes; puree in batches in blender or food processor until smooth. Return to saucepan and cook over medium-low heat until just bubbling. Remove from heat. Ladle into soup tureen or individual soup plates; swirl in cream and garnish with reserved peas. Serve hot, warm or cold with soft rolls, tomato and cucumber salad and a crisp, dry white wine.

Spaghetti with Peas, Asparagus and Arugula

YIELD: Makes 6 to 8 servings

INGREDIENTS: 

3 tablespoons unsalted butter

1 tablespoon extra virgin olive oil

2 bunches scallions, top third discarded, bottom two-thirds cleaned and sliced

1 garlic clove, minced

½ cup dry white wine

¾ cup chicken broth

1½ bunches asparagus, cleaned, bottoms of stalks removed, then sliced diagonally cut into 1-inch pieces

1 pound thin spaghetti

One 10-ounce package frozen peas, thawed, or 2 cups shelled fresh peas

One 10-ounce package sugar snap  peas, thawed

1 bunch arugula, washed, destemmed and torn into bite-size pieces

1/3 cup extra virgin olive oil

1½ cups grated Parmesan cheese

Salt and freshly ground pepper, to taste

DIRECTIONS: 

In large skillet over medium heat, melt butter; add olive oil. Add scallions and cook over medium heat until they are soft but not brown, about 8 minutes. Add garlic and white wine; cook over medium-high heat until liquid is reduced to a glaze. Add chicken broth, stir and remove from heat. 

Cook asparagus in large pot of boiling water until tender but still bright green about 5 minutes. Remove asparagus from water and set aside; reserve water. Cook spaghetti according to package directions in water from asparagus, then drain and place in skillet with scallions, garlic, wine and broth. Heat over medium low flame and toss with thawed peas and snap peas. 

When heated through, place in large pasta bowl, and toss with arugula, the one-third cup oil, Parmesan cheese and salt and pepper. Serve warm with crusty Italian bread, sliced tomatoes and mozzarella cheese.

Cold Basmati Rice Salad with PeasandCarrots

YIELD: Makes 8 servings.

INGREDIENTS: 

5 to 6 cups cooked basmati rice

2 cups thawed frozen peas or 2 cups cooked shelled fresh peas

2 cups cooked diced fresh carrots

2 tablespoons minced fresh mint leaves

2 scallions, finely chopped

¼ cup finely chopped fresh chives

1/3 cup extra virgin olive oil

3 tablespoons freshly squeezed    lemon juice

1 tablespoon freshly grated lemon zest

Salt and freshly ground pepper, to taste

DIRECTIONS: 

In a large bowl, thoroughly combine all ingredients. Cover and refrigerate until ready to serve. Can be served at room temperature or cold with meat, poultry or fish.

Mother’s Day is still a few weeks away, giving you plenty of time to shop around for a living plant that will typically be in bloom around second Sundays in May for many years to come.  

Three woody plants readily come to mind that happen to be 100 percent deer proof, long-lived, justifiably popular and reliably in flower May 5 to 15 on Long Island: wisteria, lilac and tree peony. Choice varieties of wisteria and lilac can be found locally at garden centers, with tree peonies less likely. You can get some terrific varieties of tree peonies via mail order, and there is still time to get an order delivered by May 13 but don’t delay.

If you buy an old home with a garden, it is very likely there will be some herbaceous peonies. Durable and long-lived, they are an old-fashioned perfumed favorite for garden and vase. But these peonies die to the ground and are cut down in early fall.

Tree peonies, on the other hand, are so-called not because they become trees but because they have woody stems: They are shrubs that do not get cut annually to the ground.

Originally cultivated a couple millennia ago in China for the medicinal use of the skin of its root, Paeonia suffruticosa is a conglomerate of wild species, primarily Paeonia rockii, that have been hybridized. Its sumptuous beauty was naturally celebrated early on by artists, scholars and monks, thereby attaining cultural and imperial significance and assimilation into Japanese court and temple too. In both China and Japan they came to represent wealth, status and female beauty:

It seemed as though there were

No flowers around them.’

The advent of grafting for propagation in the Edo period in Japan contributed to its spread, and breeding by famed nurseryman Lemoine in France in the 19th century and by the American A. P. Saunders in the 20th have resulted in scores of stunning varieties of color and form. 

They are expensive. You can chance upon cheap one- or two-year grafts, in two-gallon pots, labeled merely “red” or “white” or “pink” or “purple.” I avoid these, unless I catch one in flower from which to judge its vigor, and the price is not more than $30.

There is a wonderful source in northwest Connecticut called Cricket Hill Garden, which I have not visited. It has a beautiful website, dozens of varieties for sale, though some ship only for fall-planting, and White Flower Farm has always shown great judgment in the quality of its plant selection ­— they have six gorgeous varieties still available.

These will be shipped bare-root. You could pot it up for presentation. Avoid a water-logged site for planting. All peonies, herbaceous or woody, will thrive in full sun. Dappled shade is often recommended for tree peonies, and this is indeed preferable because the flowers will last longer, which is not many days by the way, even in the best circumstances. If your soil is compacted you must loosen a square yard a foot deep and, of course, amend with compost if it isn’t loamy. 

If planted in good soil in a good location your gnarly little grafted tree peony will make modest growth this year; will present one or two flowers next year; and will, in five years, become a yard high and wide, with at least a dozen fat buds, and will for many more years to come be a perennial highlight in the gardening year.

Lifestyle changes can reduce COPD exacerbations

COPD, or chronic obstructive pulmonary disease, is the third leading cause of mortality in the United States (1), although it’s not highlighted much in the layman’s press.

COPD is an umbrella term that includes emphysema, chronic bronchitis of more than three months for two consecutive years and/or chronic obstructive asthma. It is an obstructive lung disease that limits airflow. The three most common symptoms of the disease involve shortness of breath, especially on exertion, production of sputum and cough. This disease affects 6.7 percent of the U.S. population (2).

It tends to be progressive, meaning more frequent and severe exacerbations over time. Since it is a devastating and debilitating chronic disease with no cure, anything that can identify and prevent COPD exacerbations, as well as comorbidities (associated diseases), is critically important.

What are the traditional ways to reduce the risk of and treat COPD exacerbations? The most important step is to stop smoking, since 80 percent of COPD is related to smoking. Supplemental oxygen therapy and medications, such as corticosteroids, bronchodilators (beta-adrenergic agonists and anticholinergics) and antibiotics help to alleviate symptoms (3).

One of the underlying components of COPD may be chronic inflammation (4). Therefore, reducing inflammation may help to stem COPD exacerbations. There are several inflammatory biomarkers that could potentially help predict exacerbations and mortality associated with this disease, such as interleukin-6 (IL-6), C-reactive protein (CRP), leukocyte (white blood cell) count and fibrinogen (a clotting factor of the blood).

How do we reduce inflammation, which may contribute to exacerbations of this disease? Some drugs, such as statins, work partially by reducing inflammation. They may have a role in COPD. Lifestyle changes that include a high-nutrient, anti-inflammatory diet and exercise may also be beneficial. Let’s look at the evidence.

Biomarkers for inflammation

In a recent population-based study with over 60,000 participants, results show that as three biomarkers (CRP, leukocyte count and fibrinogen) were elevated, the risk of COPD exacerbation increased in a linear manner (5). In other words, the risk of frequent exacerbation increased 20, 70 and 270 percent within the first year as the number of elevated biomarkers increased from one to three, compared to patients who did not have biomarker elevations.

As time progressed beyond the first year of follow-up, risk exacerbation continued to stay high. Patients with all three biomarkers elevated for longer periods had a 150 percent increased risk of frequent exacerbations. These predictions were applicable to patients with stable and with mild COPD.

In an observational study, results showed that when the biomarker IL-6 was elevated at the start of the trial in stable COPD patients, the risk of mortality increased almost 2.7-fold (6). Also, after three years, IL-6 increased significantly. Elevated IL-6 was associated with a worsening of six-minute walking distance, a parameter tied to poor physical performance in COPD patients. However, unlike the previous study, CRP did not show correlation with increased COPD exacerbation risk. This was a small trial, only involving 53 patients. Therefore, the results are preliminary.

These biomarker trials are exciting for their potential to shape treatments based on level of exacerbation risk and mortality, creating more individualized therapies. Their results need to be confirmed in a randomized controlled trial (RCT). Many of these biomarkers mentioned in the two trials are identifiable with simple blood tests at major labs.

Statin effect

Statins have been maligned for their side effects, but their efficacy has been their strong suit. An observational trial showed that statins led to at least a 30 percent reduction in the risk of COPD exacerbations, with the effect based on a dose-dependent curve (7). In other words, as the dose increased, so did the benefit.

Interestingly, even those who had taken the statin previously saw a significant reduction in COPD exacerbation risk. The duration of statin use was not important; a short use of statins, whether presently or previously, had substantial benefit. 

However, the greatest benefit was seen in those who had been on a medium to high dose or were on the drug currently. The researchers believe that the mechanism of action for statins in this setting has to do with their anti-inflammatory and immune-modulating effects. This was a retrospective (backward-looking) study with over 14,000 participants. We will need a prospective (forward-looking) study and an RCT to confirm the results.

Exercise is beneficial for almost every circumstance, and COPD is no exception. But did you know that a pedometer might improve results? In a three-month study, those with mild COPD were much more successful at achieving exercise goals and reducing exacerbations and symptoms when they used pedometers, compared to the group given advice alone (8). Pedometers gave patients objective feedback on their level of physical activity, which helped motivate them to achieve the goal of walking 9,000 steps daily. This is a relatively easy way to achieve exercise goals and reduce the risk of COPD exacerbations.

When exercising, we are told to vary our exercise routines on a regular basis. One study demonstrates that this may be especially important for COPD patients (9). Results show that nonlinear periodization exercise (NLPE) training is better than traditional routines of endurance and resistance training in severe COPD patients. The goal of NLPE is to alter the time spent working out, the number of sets, the number of repetitions and the intensity of the workout on a regular basis.

This study was randomized, involved 110 patients and was three months in duration. Significantly more severe COPD patients achieved their exercise goals using NLPE rather than the traditional approach. The group that used NLPE also had an improved quality of life response. The researchers believe that compliance with an NLPE-type program is mostly likely going to be greater because patients seem to enjoy it more.

Chronic inflammation may play a central role in COPD exacerbation. Nonspecific inflammatory biomarkers are potentially valuable for providing a more personalized approach to therapy. Drugs that can control inflammation, such as statins, show promise. But don’t forget the importance of lifestyle changes, such as quitting smoking and committing to an exercise regimen that is varied and/or involves the use of a pedometer. And potentially a high-nutrient, anti-inflammatory diet will also contribute positively to reducing the frequency and severity of COPD exacerbations.

friend recently told me she’s pregnant with her first child. She sounded thrilled and anxious. She is, as I’ve known for years, incredibly organized and efficient. She has been a standout in her job for several years.

“What’s the concern?” I asked.

“Everything,” she giggled.

As my children take one standardized test after another, I thought perhaps I would share a test-format version of what to expect when you’re expecting. No. 2 pencils ready? OK, let’s begin:

Question 1: Before the baby is born, you should:

a. Panic buy everything, including a crib and six months worth of food and clothing. You never know if you’ll be trapped in your house without access to the outside world.

b. Sleep as much as you can because the days of sleeping at your leisure are over.

c. Read everything you can about parenting and the delivery, and then realize that every process, including childbirth, can go off script.

d. Don’t tell anyone because people will write about you.

Question 2: When people give you advice, you should:

a. Write everything down because friends, family and strangers always know better and will enlighten you with wisdom that far exceeds that which you’d get on a fortune cookie.

b. Nod politely, say, “That’s a great idea,” and wonder what to eat for dinner.

c. Pretend your phone is ringing.

d. Ask them how many Nobel prizes their children have won.

Question 3: Taking Lamaze classes can be helpful because:

a. It allows you to meet parents who are older than you.

b. It allows you to practice breathing together because sometimes parents forget to breathe.

c. It’s so relaxing that you can doze off without punishment.

d. It gives you a sense of control that you’re unlikely to have in the actual moment.

Question 4: People generally love other people’s children unless:

a. They are sitting on a plane near them.

b. They have to do something for them.

c. The children are crying constantly and they don’t know why.

d. The children have dropped or broken something.

Question 5: Parents can be so tired in the early stage that they forget:

a. To take pictures of everything.

b. To feed themselves.

c. To go to the bathroom when they need to.

d. To revel in a new baby smell that will change into something much more challenging to the nose within a year.

Question 6: When you have a baby, it’s a great idea to:

a. Change jobs.

b. Move to a new city.

c. Start attending a new and rigorous educational course.

d. All of the above, because you’ll never have a chance to juggle more challenges at the same time than when a baby is born.

Question 7: The families of the father and mother are likely to:

a. Always agree on everything you should do for the child.

b. Never agree on anything you should do for a child.

c. See evidence of their family’s genes in the child.

d. Put small differences aside and enjoy the moment when they share a new relative.

Question 8: Once you have a child, you will:

a. Be thrilled when young children come over to play with your child.

b. Be worried that the young children who come over are sniffling.

c. Want everyone to bathe in Purell sanitizer before coming near your child.

d. Not be like me and will relax when people sneeze across the room.

Question 9: You know you’ve had a great day with your child when:

a. You keep replaying something he or she said or did as you’re preparing to sleep.

b. You actually go to sleep instead of passing out with Oreo cookie crumbs in your mouth.

c. You and your spouse are laughing, quietly, for hours before you go to sleep.

d. You can’t wait to start the next day.

Question 10: Parenting is:

a. Awesome.

b. Terrifying.

c. Exhausting.

d. All of the above.
