It’s been a long time since I took a child to a playdate or to the first day of a kindergarten class. And yet, I felt as if I had gone through a time warp recently when my daughter, who is home for spring break, and I took our three-year-old dog Bear for his second visit to a dog run.

While I’m sure many dog owners are familiar with the process, I found the collection of dogs circling trees, bushes and owners fascinating and familiar.

When we arrived, several dogs played in groups of shifting sizes while their owners, like anxious parents hoping their children play well together, stood by, observing the action and preparing to intercede.

Dog owners looked back and forth at my daughter and me, trying to figure out which of the collection of pets straight of a Dr. Seuss book filled with colorful illustrations of dogs of all shapes and sizes was ours.

That process isn’t as obvious as the genetics of trying to match the faces of young children with the parents standing by, waiting for the bell to ring and a teacher to bring their children inside.

Like protective parents, many of the dog owners watched their pets carefully, not only to make sure they were behaving, but also to ensure that none of the other dogs was threatening them.

Some dog owners shared stories about their dogs, much as my children’s classmates had done over 15 years ago, talking about what their dogs like to do and how eager they are for their dogs to get out all their energy now, so they’ll sleep well. Just as it does for young children, a day of healthy activities means a good night’s sleep.

A medium-sized dog paused in a puddle, stomping in the squishy mud. Her owner raced over and barked at Roxy to “stop,” annoyed that her paws looked like they had brown booties.

Meanwhile, a giant dog with the name Zeus written on a horse collar lumbered from one group to another, his head held higher than other dogs who came up to his shoulder.

Bear shifted from one group to another, awed by the athletic prowess of two huge dogs that vaulted onto a picnic table. 

At one point, Bear trotted to the other extreme end of the park, almost out of sight. I whistled for him and, despite his tendency to ignore me at home, he immediately picked up his head and pitched his ears forward. I signaled for him to come back and, to my amazement, he jogged the length of the field, where my daughter and I pet him appreciatively.

While Bear played with the other canines, he also visited every pet owner, thrusting his head towards their knees and staring up at them with his best “I-know-you’re-a-dog-person-so-please-pet-me” face.

An aggressive dog barked and nipped at the others who had been playing peacefully. After the newcomer lunged at Bear three times, he trotted to the exit, glancing over his shoulder periodically to make sure we were coming. We obediently followed.

Once we were near our car, an unleashed dog raced around the lot, as his owner shouted for Oliver repeatedly to come back and to stay away from cars moving slowly enough to avoid loose dogs.

As we drove home, with our dog panting from the exertion in the back seat, I glanced at our daughter and appreciated the brief trip down memory lane when we brought her home from playing with her peers.

Our dog has no intention of trekking off to college, even if he’s eager to explore the world of our neighbors’ houses, where the grass sometimes seems greener.

Like a hand reaching out from its watery grave, the stern of the ship with the name “Endurance” became visible in the underwater drone’s searching beacon of light. A century after the ice crushed and sank the vessel, along with the hopes of explorer Ernest Shackleton and his crew for being the first to walk across Antarctica from sea to sea via the South Pole, the biggest shipwreck discovery since the Titanic connected us with those men a century ago. For many of us, the find was thrilling.

The three-masted ship is remarkably preserved in 10,000 feet of water below the surface ice, and from the photos, even the spokes on the wheel in the stern are hauntingly intact. Armed with the latest undersea equipment, marine archeologists, engineers and scientists, using the last data recorded when the ship sank, were able to find the wooden Endurance, survivor of one of the most heroic expeditions in history, at the bottom of the Wendell Sea near the Antarctica Peninsula. The Falklands Maritime Heritage Trust’s group Endurance 22 announced the news. The darkness and frigid temperatures had made such search efforts in the past impossibly difficult but also created an inhospitable environment for bacteria, mites and wood-eating worms that might have devoured the ship. Instead it stands at attention since 1915 on the sea floor.

After the ship sank, Shackleton and his crew of 28 loaded food and anything else they could into three lifeboats and set up camp on ice floes, and when those disintegrated, camped on Elephant Island.

Recognizing that they had somehow to get help if they were to survive, Shackleton, his captain, Frank Worsley and four other carefully selected men sailed across 800 miles of treacherous waters in a 22-foot boat to the nearest place of habitation, a remote whaling community on the island of South Georgia. Once they arrived, they had to scale steep mountains to get to the station on the other side. Shackleton’s decisive and heroic leadership ultimately saved the entire crew and is studied in business schools and management programs to this day. His planning and improvisation made the escape possible.

Shackleton died in 1922. Curiously the wreck’s discovery happened exactly 100 years to the day that Shackleton was buried. And while Endurance was photographed and filmed, nothing was removed or disturbed, and it is protected as an historic monument.

An Anglo-Irishman, Sir Ernest Shackleton was born in County Kildare, Ireland, and moved with his family to south London. His story seems a fitting way to celebrate St. Patrick’s Day. He led three different English expeditions to the Antarctic, walked to within 97 miles of the South Pole during the previous Nimrod expedition of 1907-09, and climbed Mt. Erebus, the most active Antarctic volcano. For those feats, he was knighted by King Edward VII on his return. Ultimately he led a final expedition in 1921 but died of a heart attack while his ship was moored in South Georgia. He is buried there. 

Despite the fact that he was largely unsuccessful in business ventures and died heavily in debt, Shackleton was voted eleventh in a BBC poll of the 100 Greatest Britons in 2002. He was to be the one others prayed to have lead them when under extreme circumstances.

The saga of Ernest Shackleton and the Endurance captured my imagination when I worked for Editor Alfred Lansing at Time Inc. I was 22 and had never met anyone quite like Al before. A volunteer in the Navy when he was 17 (he lied about his age and somehow got in), Al had a reddish-blond crew cut, bright blue eyes, a huge smile and a tattoo on his right forearm well before tattooes were a common occurrence. He smoked unfiltered Lucky Strikes, was one of the best storytellers I had ever met, and wrote adventure stories on the side for what were then called men’s magazines.

It was Alfred Lansing who wrote the book “Endurance: Shackleton’s Incredible Voyage” four years earlier which had received a National Book Award nomination. Listening to him tell the story, I was hooked for life on that adventure and the marvel of Shackleton’s leadership. Sadly, both men died at an early age.

YIELD: Serves 6

INGREDIENTS: 

1 tablespoon extra-virgin olive oil

1 1/4 pounds beef, top round, cut into 3/4-inch pieces

3 cloves garlic, minced

salt, to taste

pepper, to taste

1 medium onion, coarsely chopped

3 medium carrots, peeled and cut into 3/4-inch pieces

2 medium parsnips, cut into large chunks

3 cups low-fat, reduced-sodium beef broth

4 medium russet potatoes, peeled and cut into large chunks

1 tablespoon chopped fresh rosemary

1 leek, coarsely chopped

2 tablespoons chopped fresh parsley

DIRECTIONS:

In a large pot over medium-high heat, heat oil. Add beef and garlic. Cook, gently stirring until meat is evenly browned. Season with salt and pepper. Add onion, carrots and parsnips. Cook 3 to 4 minutes. Stir in broth and bring to a boil. Reduce heat to low and simmer about 75 minutes, or until meat is tender. Stir in potatoes and simmer another 30 minutes. Add rosemary and leeks. Continue to simmer, uncovered, until potatoes are tender. To avoid potatoes from falling apart, do not overcook. Serve hot and garnish with parsley, if desired.

This potato bread is just as easy to make as a soda bread, in fact it’s pretty much the same recipe with the addition of potatoes and the potatoes help keep this bread so nice and moist and tender giving it a heavenly texture! 

YIELD: Makes 1 bread

INGREDIENTS: 

2 medium or large russet potatoes

1 egg

1 egg white

1/3 cup canola oil

3/4 cup milk

2 tablespoons green onion, minced

1/2 teaspoon caraway seeds

3 1/4 cups all-purpose flour, plus additional for dusting and kneading

1 1/2 tablespoons baking powder

1 teaspoon salt

DIRECTIONS:

Heat oven to 375 F. Peel potatoes. Slice one potato and boil in saucepan 15 minutes, or until tender. Remove potato from saucepan into large bowl. Mash potato then set aside. Grate second potato onto cloth. Wring potato in cloth to remove excess water. Add grated potato to mashed potato in large bowl. Add egg, egg white, oil, milk, onion, caraway seeds, 3 1/4 cups flour, baking powder and salt. Stir with wooden spoon until mixture is soft and sticky. 

Turn dough onto floured surface. Adding flour as needed, knead dough to form 8-inch round shape with slight dome. Place dough onto baking sheet lined with parchment paper. Cut large “X” on top of dough about 1/2 inch deep. Bake 55 minutes until golden brown. Cool on wire rack 1 hour before serving.

YIELD: Makes 2 pies (9 inches each)

INGREDIENTS: 

3 3/4 cups heavy whipped cream, divided

8 ounces cream cheese, softened

1 1/4 cups, plus 2 tablespoons, powdered sugar, divided

5 drops green food coloring

1 1/4 teaspoons vanilla extract, divided

1/4 teaspoon mint extract

1 bag mint chocolate candies, chopped, divided

2 chocolate cookie crusts (9 inches each)

1 bag mint chocolate candies

DIRECTIONS:

To make filling: In bowl of stand mixer, whisk 2 1/2 cups heavy cream until stiff peaks form. Transfer to bowl. In separate stand mixer bowl, beat cream cheese on high 2 minutes. Gradually add 1 1/4 cups powdered sugar and green food coloring; mix until smooth. Add 1/4 teaspoon vanilla and mint extract; mix well. Fold prepared whipped cream into cream cheese mixture. Fold 1 cup chopped mint chocolate candies into filling.

To make frosting: In bowl of stand mixer, whisk remaining heavy cream and remaining powdered sugar. Add remaining vanilla extract and mix until stiff peaks form. Pour filling into crusts and smooth tops. Fill decorating bag with frosting and pipe thick band around edges of pies. Then pipe circle dollops evenly around edges of pies. Scatter 1 cup chopped mint chocolate candies in middle of pies. Place whole mint chocolate candies into each dollop of frosting around edges of pies. Refrigerate until firm, 5 to 6 hours, before serving

The response of Long Islanders to help Ukrainians has been encouraging.

On March 3, Suffolk County Legislator Stephanie Bontempi (R-Centerport) and Dr. David Buchin headed up the Long Island Ukrainian Emergency Response Drive at their offices, located at 224 Wall St. in Huntington. Local elected officials and community members joined them to gather items such as blankets, medical supplies, sleeping bags and other essentials to send to the embattled country.

On Monday, Suffolk County Sheriff Errol Toulon Jr. (D) announced the addition of nearly 450 pieces of bulletproof body armor from his office to the items already collected in Huntington. The College Hunks moving company assisted in delivering the items to the New York City processing center and from there the items will be shipped to Ukraine.

Donation drives similar to those of Bontempi and Buchin along with rallies to support Ukrainians have taken place all over the TBR News Media coverage areas. The message sent across the globe is that we stand with the country in their fight against Russia.

TBR News Media readers have also reached out with letters to the editor condemning Russia’s actions, and another reader shared the struggles his Ukrainian family faced in the past. He hoped that history would not repeat itself, but he was ready to help any relative who needed his assistance.

The last few years have been divisive ones for the United States. Between the political divide and our battle against COVID-19, it seemed as if we couldn’t look past our own borders. Now we see Americans uniting once again for a cause.

It’s a shame it took such a tragedy to bring us all together. It may have taken a war, but Americans seem once again united behind a common theme: the cause of freedom, democracy and the right of a free people to self-determination.

As U.S. Rep. Tom Suozzi (D-NY3) said on March 3 in front of the offices of Bontempi and Buchin, “We have to follow the old idea that partisanship stops at the water’s edge. We have to all hold together like we are today.”

As we watch Long Islanders work together to help people struggling on the other side of the globe, let’s remember what it’s like to be united as Americans, not only past the water’s edge but on our own island.

■ Home Depot on Crooked Hill Road in Commack reported a shoplifter on March 4. A man allegedly stole multiple firestick remotes for a total value of $440.

■ Walmart on Crooked Hill Road in Commack called the police on March 2 to report that a man allegedly stole a car seat worth $180 from the store.

■ A resident on Chardonnay Road in Commack reported that a man allegedly entered this unlocked vehicle on March 1 and removed a pair of Persol prescription sunglasses valued at $900.

A  man who allegedly threw a rock through a window at the Commack Smoke Shop in Commack on March 1 at approximately 1:20 a.m. and stole cash. He fled the scene on foot before getting in a white SUV.

■ A resident on Princeton Drive in Dix Hills reported that a man allegedly entered his unlocked vehicle on March 2 and stole clothing and sunglasses.

■ Target on East Jericho Turnpike in Huntington Station reported a shoplifter on March 3. A man allegedly stole a Polaroid camera worth $150.

■ Sno-Haus on West Jericho Turnpike in Huntington Station reported a shoplifter on March 2. A man allegedly stole men’s ski jackets valued at $2590.

■ A resident on Beverly Road in Huntington Station called the police on March 3 to report that someone had entered their unlocked vehicle and stole a wallet containing cash and credit cards from the center console.

■ A resident on Neslo Drive in Nesconset called the police on March 3 to report that someone had stolen a wooden carousel horse valued at $250 from a barn on his property the night before.

Port Jefferson

■ C & C Collision on Texaco Avenue in Port Jefferson called the police on March 1 to report that someone stole a customer’s 2020 Nissan Pathfinder from their parking lot after the keys were inadvertently left on the dashboard. The vehicle was valued at $25,000.

Port Jefferson Station

■ A  person left their 2011 Toyota RAV4 running and unlocked while shopping at the 7-11 on Route 112 in Port Jefferson Station on Feb. 28. When he came out of the store, the vehicle was gone.

St. James

■ Exxon on Middle Country Road in St. James reported a petit larceny on March 4. A customer allegedly stole $138 worth of printed lottery tickets.

Selden

■ Bob’s Store on Middle Country Road in Selden reported a shoplifter on March 2. A man allegedly took 4 pairs of Timberland Pro boots out of the store without paying for them. The total value of the merchandise was approximately $600.

■ Items were stolen from two cars parked at Panera Bread on Middle Country Road in Selden on March 3. The rear passenger windows were broken. A Dell laptop, backpack, various tools and notebook agenda were taken.

■ Aldis on Middle Country Road in Selden reported a shoplifter on March 3. A woman allegedly stole assorted items valued at approximately $300.

■ Famous Footwear on Middle Country Road in Selden called the police on Feb. 28 to report a shoplifter. A woman allegedly stole two pairs of Nike Court High Top sneakers valued at approximately $180.

■ Target on Middle Country Road in Selden reported a petit larceny on Feb. 28. A woman allegedly stole assorted bedding items, cosmetics and clothing valued at approximately $200.

Smithtown

■ A resident on Landing Meadow Road in Smithtown reported on March 2 that a man drove up to their house and stole their empty recycling bin. The item is valued at $30.

■ Costellos’s Ace Hardware on East Main Street in Smithtown called the police on March 2 to report that a man allegedly stole a DeWalt Combo Drill valued at $450.

■ A man walking home from work on Main Street in Smithtown was robbed by two men who pulled up in a vehicle on Feb. 27. The man handed over a silver chain, cell phone and his wallet.

South Setauket 

■ Stop & Shop on Pond Path in South Setauket reported a shoplifter on March 2. A man allegedly stole assorted merchandise worth approximately $400.

Not sure if planetary scientists can explain why, when the earth was forming, it became tilted about 23.5 degrees off a perfect perpendicular axis to its orbital plane around the Sun. However, they can offer an unequivocal statement of fact that this planetary quirk is the reason for the portfolio of seasons we enjoy. And now, as has been often true for more than four and one-half billion years, when the planetary axis that runs through the North Pole points away from the Sun, the Northern Hemisphere receives weaker, more obtuse rays of sunshine, resulting in the colder temperatures of winter. 

Today, as they have for millenia, countless number of plants and animals have responded in their own species-specific ways to survive this most challenging of seasons.

A discussion about the pervasive effects of winter on nature cannot happen without talking about another word that begins with the letter “w” and ends in an “r” — water. Water, or more particularly the fact that it becomes ice at 32 degrees, has had profound impacts in shaping the response of organisms to winter. As water becomes ice, it’s no longer available to plants, making winter, in effect, a five to six month long drought. The response of deciduous trees to no available water? To shed their leaves that are water loss structures and become dormant. How do evergreen or coniferous trees, which obviously keep their leaves, tolerate the winter’s loss of available water? Their small leaves with waxy coatings are highly effective at retarding water loss. They simply use little water in the winter.

How else does ice affect species? Ducks, geese and swans that depend upon open freshwater ponds and lakes to feed need to move in the event their ponds freeze over. Same with kingfishers and other fish-eating birds. This “freezing over” occurs because ice, by rare virtue of being less dense than liquid water, floats on the surface of the surface of the pond or lake, rather than freezing at the bottom which would happen if ice were denser than water, which is the norm with so many other liquids. This unusual, almost unique, attribute — of solid water (ice) being lighter than liquid water — has played a hard to overstate role in allowing for life on earth to evolve and flourish, for if ice were denser the entire waterbody would freeze solid to the detriment of everything living in it.

Unlike immobile species such as trees, mobile species (i.e. animals that fly!) adapt to winter by simply leaving it behind, winging to warmer climates where they can continue to feed (some species living a perpetual summer existence!). Such is the case with dozens of bird, bat, and insect species that migrate vast distances to find climates and associated food supplies to their liking. 

For example, ospreys depart from northern latitudes because the fish they depend upon are unavailable, either because they can’t access them due to ice or because salt-water fish move into deeper water where they cannot be caught, forcing ospreys to move to habitats within climates where food is available. Insect-eating songbirds move off too but in their case because of the disappearance of available insects.

Mobile species that don’t migrate employ a variety of other strategies to survive the winter. A perhaps most well-known — but relatively rare — strategy is hibernation. Hibernating mammals species adapt to winter by so reducing their energy and water needs they can tide over from autumn to spring. 

The woodchuck (aka groundhog) is the best known hibernator. Curled in an underground den, a hibernating woodchuck’s heart beat drops from about 100 beats per minute to four to five and its body temperature more than half, from about 99 degrees to 38-40 degrees. Bats that don’t leave for warmer climates also hibernate. All hibernating species depend upon stores of fat, built up from continued feeding in the autumn, as the energy source to make it through winter.

Just below hibernation is torpor, a physiological state in which the animal’s metabolism, heart and breathing rates are reduced but which still allows it to be alert enough to react to danger. Chipmunks (and bears) are well known examples and speaking of chipmunks — they illustrate another common practice of many animals to make it through the winter — storing up food in winter larder. Beavers do the same by bringing leaf-laden branches underwater, a wet refrigerator of sorts, where food is safely ensconced.

Regulated hypothermia is yet another adaptation to surviving winter. In this case, the animal reduces its temperature while sleeping, enabling it to reduce the amount of heat lost to the air overnight. Black-capped chickadees are a well-known example. During the winter chickadees drop their temperature each night from about 108 degrees to the mid-90’s by employing this practice. They also seek sheltered places like tree cavities (another reason to let dead trees stand if they pose no safety risk) and dense vegetation where they can stay warmer.

Cold blooded animals such as reptiles and amphibians make it through winter by experiencing their own form of hibernation — an activity known as brumation. Like with warm blooded animals, brumating reptiles and amphibians significantly reduce their heart, breathing and general metabolic rates. Some species, like diamondback terrapins, are spared the full brunt of winter by brumating in the muddy bottoms of bays, harbors, and river mouths where the temperature never drops below freezing. Not so with the wood frog, a wide ranging amphibian that in March emerges to explosively breed in woodland vernal pools around Long Island. 

Wood frogs are known to freeze solid, becoming ‘frogsicles’ during the winter and getting as close as a live animal can get to being dead. As autumn slides into winter, wood frogs undergo a several-step physiological process whereby water is pulled out of cells and is stored between them. This movement of water from inside the cell to sites between the cells occurs because water stored within the cell, if frozen, would form sharp ice crystals, likely puncturing cell membranes, thereby destroying the cell. 

The frog’s metabolism, breathing, and heartbeat stop and the frog remains in a state of animated suspension for many weeks. Come the Spring though, and this very dead looking frog slowly comes back to life, none worse for the wear. It becomes active and vibrant, soon filling small wetlands with its quacking duck calls.

For the lover of nature and the outdoors there are gifts of winter: clear night skies; falling snow and geometric snowflakes; frost patterns on windows; sledding and hot chocolate (or for some adults mulled apple cider spiked with a little spirit!); no leaves to hide bird nests or tree buds, like those of American Beech, which Henry David Thoreau called “the spears of Spring”; the dried stalks of countless wildflowers; the “pen and ink” quality of landscapes; the presence of snowy owls and snow buntings at the beach; or the arrival of many types of ducks and geese. Winter is not an absence of summer; it is a season complete and whole to itself.

Perhaps this article won’t serve to change your thinking if you’re among the crowd of people who find winter to be their least favorite season. Still, winter illustrates so clearly and compellingly the fine-tuned lives of so many plants and animals, each unique to this time of cold, lives that have developed, over eons of time, countless strategies to make it through the unrelenting cold and sparse food supplies of the winter season.

An over-the-counter stomach-soothing medication may also relieve some of the symptoms of mild to moderate COVID-19.

In a study recently published in the journal Gut, Cold Spring Harbor Laboratory Assistant Professor Tobias Janowitz and a team of collaborators at CSHL and The Feinstein Institutes for Medical Research at Northwell Health demonstrated that Famotidine, the active ingredient in Pepcid, shortened the duration of symptoms for a diverse patient group of adults soon after developing COVID-19 symptoms.

In a placebo-controlled study, people taking 80 milligrams of Famotidine three times a day reported that symptoms such as headaches declined after 8.2 days, compared with 11.4 days for patients who were taking the placebo.

“We think that the results are preliminary, but encouraging,” Janowitz explained in an email. 

The research, which included 55 volunteers, may offer health care providers another tool to help treat mild to moderate cases of COVID-19. In the clinical study, the use of Famotidine helped reduce a potentially overactive inflammatory response without suppressing the immune system’s efforts to ward off the virus.

Participants in the study received Famotidine or placebo pills along with a host of instruments they could use at home to gather clinical data about themselves, including a cellular activated Apple iPad, a scale, thermometer, fitness tracker, spirometer to measure air flow in and out of the lungs and a pulse oximeter, which measured oxygen levels by taking a reading over a person’s fingernails.

The protocol for the study allowed volunteers to stay home, where they gathered results from the instruments and reported on their health and any symptoms they felt. Technicians came to the home of each volunteer on the first, seventh, 15th, and 28th days after entering the clinical trial.

Researchers and doctors involved in the analysis of the effectiveness of COVID believe this remote approach to participating in clinical trials could prove a safe and effective way to conduct research for other diseases.

“In today’s virtual world, our clinical trial strategy has significant implications for how to study new drugs in patients at home,” Dr. Kevin Tracey, president and CEO of the Feinstein Institutes, explained in a Cold Spring Harbor Laboratory news brief.

Janowitz added that other studies could also use testing protocols at home, including for other diseases. “We are looking forward to employing it to help develop better treatments for people with cancer,” which is the disease at the center of his research, he explained.

The CSHL Assistant Professor focuses on the whole body response to cancer, although many of the biological considerations are transferable to other diseases.

Pivot to COVID

According to Janowitz, “It was relatively easy for us to pivot to COVID research when it was a global area of unmet need.” 

The researchers chose Famotidine because of encouraging studies and from a case series, Janowitz explained. They also found a potential mechanism of action where Famotidine blocked the H2 receptor, which encouraged them to move to a phase 2 randomized clinical trial.

The researchers were pleased that the participants in this small trial included people from a range of ages and ethnic groups. Nearly two thirds of patients, who were 18 years and older, were from black, mixed-race or Hispanic communities.

“Patients with different ancestry may have different responses to this disease,” Janowitz explained. “It helps to learn about the generalizability of the results.”

In a CSHL news brief, Nicole Jordan-Martin, executive director for New York City Health + Hospitals, added that “accessible, safe and low-cost outpatient treatment options are a priority in our global efforts to combat COVID-19.” Northwell and New York City Health + Hospitals provided care for the communities most in need of support for New York City, she added.

The collaborators were also encouraged by their teamwork.

“Our institutions worked extremely well together to face challenges the pandemic posed, like offering digital solutions and reaching populations who struggled for access to care,” Dr. Christina Brennan, vice president of clinical research at the Feinstein Institute and co-investigator of the trial, said in the news brief. 

“From screening patients to organizing home delivery of the equipment and medication, this sets a new model for future trials and convenience for participants.”

Janowitz described the safety profile of Famotidine as “excellent” and said it “appears to have few interactions with other drugs and very few side effects in general.”

To be sure, Janowitz cautioned doctors and patients not to stock up on Famotidine before researchers conduct additional studies.

“Our trial is not conclusive and an early phase clinical trial (phase 1 or 2) is not sufficient to inform clinical practice,” he wrote.

Additionally without further study, researchers don’t know the best potential dose and dosing interval for this possible treatment. At this point, they know how long the drug stays in the blood and the strength of its binding to its receptor.

A dose of 20 milligrams per day or less may be too little to achieve an effect, but “we do not know this for certain,” Janowitz explained.

While researchers agreed that further studies were necessary to answer key questions, they believed that the results from this research could provide fodder for studies outside of the COVID world.

“It is possible that sustained inflammation contributes to illness in other contexts and changing this inflammation would be beneficial,” Janowitz wrote. “This will have to be explored separately. Importantly, the methods used in this trial are also transferrable, so we have learned a lot of important information” from this research.

Improving calorie quality makes a difference

The road to weight loss, or even weight maintenance, is complex. Many things influence our eating behavior, including food addictions, boredom, lack of sleep and stress. This can make weight management or weight loss very difficult to achieve.

Unfortunately, awareness of a food’s caloric impact doesn’t always matter, either. Studies assessing the impact of nutrition labeling in restaurants gave us a clear view of this issue: knowing an item’s calories either doesn’t alter behavior or encourages higher calorie purchases (1, 2).

Does this mean we are doomed to acquiesce to food temptation? Actually, no: It is not solely about willpower. Changing diet composition is more important.

In my clinical experience, increasing the quality of food has a tremendous impact. Foods that are the most micronutrient dense, such as plant-based foods, rather than those that are solely focused on macronutrient density, such as protein, carbohydrates and fats, tend to be the most satisfying. In a week to a few months of focusing on micronutrients, one of the first things patients notice is a significant reduction in cravings. But don’t take my word for it. Let’s look at the evidence.

Refined carbohydrates

Many of us know that refined carbohydrates are not beneficial. Worse, however, a randomized control trial (RCT) showed refined carbohydrates may cause food addiction (3). Certain sections of the brain involved in cravings and reward are affected by high-glycemic foods, as shown by MRI scans of trial subjects.

Study participants consumed a 500-calorie shake with either a high-glycemic index or a low-glycemic index. They were blinded (unaware) as to which they were drinking. The ones who drank the high-glycemic shake had higher levels of glucose in their blood initially, followed by a significant decline in glucose levels and increased hunger four hours later. The region of the brain that is related to addiction, the nucleus accumbens, showed a spike in activity with the high-glycemic intake.

According to the authors, this effect may occur regardless of the number of calories consumed. Granted, this was a very small study, but it was well designed. High-glycemic foods include carbohydrates, such as white flour, sugar and white potatoes. The conclusion: Everyone, but especially those trying to lose weight, should avoid refined carbohydrates. Calorie composition matters.

Comparing macronutrients

We tend to focus on macronutrients when looking at diets. These include protein, carbohydrates and fats, but are these the elements that have the greatest impact on weight loss? In an RCT, when comparing different macronutrient combinations, there was very little difference among groups, nor was there much success in helping obese patients reduce their weight (4, 5). Only 15 percent of patients achieved a 10 percent reduction in weight after two years.

The four different macronutrient diet combinations involved overall calorie restriction. In addition, each combination had either high protein, high fat; average protein, high fat; high protein, low fat; or low protein, low fat. Carbohydrates ranged from low to moderate (35 percent) in the first group to high (65 percent) in the last group. This was another well-designed study, involving 811 participants with an average BMI of 33 kg/m², which is classified as obese.

Again, focusing primarily on macronutrient levels and calorie counts did very little to improve results.

Micronutrient deficiency

In an epidemiological study looking at National Health and Nutrition Examination Survey data, results demonstrate that those who are overweight and obese tend to be lacking in micronutrients (6). These include carotenoids, such as lutein, zeaxanthin, beta-carotene, alpha-carotene and beta-cryptoxanthin, as well as vitamin B12, folate and vitamins C, E and D.

Unfortunately, taking supplements won’t solve the problem; supplements don’t compensate for missing micronutrients. Quite the contrary, micronutrients from supplements are not the same as those from foods. With a few exceptions, such as vitamin D and potentially B12, most micronutrient levels can be raised without supplementation, by adding variety to your diet. Please ask your doctor.

Cortisol levels

The good news is that once people lose weight, they may be able to continue to keep the weight off. In a prospective (forward-looking) study, results show that once obese patients lose weight, the levels of cortisol metabolite excretion decrease significantly (7).

Why is this important? Cortisol is a glucocorticoid, which means it raises the level of glucose and is involved in mediating visceral or belly fat. This type of fat has been thought to coat internal organs, such as the liver, and result in nonalcoholic fatty liver disease. Decreasing the level of cortisol metabolite may also result in a lower propensity toward insulin resistance and may decrease the risk of cardiovascular mortality. This is an encouraging preliminary, yet small, study involving women.

Controlling or losing weight is not solely about willpower or calorie-counting. While calories have an impact, the nutrient density of the food may be more important. Thus, those foods high in a variety of micronutrients may also play a significant role in reducing cravings, ultimately helping to manage weight.

When my daughter was young, one of her favorite songs was “Old MacDonald.” Maybe she loved it because it was a song about farm animals and we lived in Manhattan, where most of our non-human wildlife consisted of squirrels and pigeons.

When she was an infant, she could make an incredibly convincing pigeon coo, thanks to hearing them all day long outside her window.

Maybe she also liked the song because, unlike Bette Middler’s “The Rose” and “One” from “A Chorus Line,” my wife and I couldn’t butcher the relatively simple melody with our unimpressive singing skills.

Anyway, she liked to say “duck” when we got to the animal on the farm. She liked ducks, or maybe the “quack quack” sound so much that she’d protest when we told her the farm already had a duck. She would say, “another duck,” to keep us quacking. The song and the quacks made car rides more palatable.

The song popped into my head recently when a friend told me that his second daughter was pregnant with their fourth grandchild. My friend has three daughters. His fourth grandchild is a girl, which means he’ll also have four granddaughters.

He was extremely pleased to share the news by email about “another granddaughter,” and he has every right to be. He is preparing for the seventh consecutive girl in his immediate family.

Then again, I couldn’t help thinking how he or the rest of the world would have felt if he had a grandson. Would that be a greater cause for celebration because they had a boy in their midst? I suspect he doesn’t, and didn’t, care.

For so many people, the gender doesn’t matter as long as the baby and mother (and somewhere in there, hopefully, the father) are healthy.

When my daughter was born, the big surprise was that I, unlike my parents or my older brother, could have a girl. As I told my wife, as the second of three boys and an uncle to two nephews, I expected to continue the male tradition.

My daughter started out proving me wrong and, thankfully, continues to do so regularly.

Once I’d broken the ‘all boys’ pattern, I was convinced my son was a girl, so, he, too, defied our expectations.

So, what is it with gender reveal parties? Is it another way to celebrate a coming birth? Is it a way to unveil one of the few mysteries left before birth?

In an era that increasingly understands and supports the transgender population, gender reveal parties seem anachronistic, celebrating a birth gender that may conflict with a person’s developing identity.

Like so many other events, such as a first birthday party that a child will never remember, a gender reveal party may be a way to celebrate the parents, giving them a chance to anticipate the coming birth and to imagine life as the parent of a boy or girl.

Gender reveal parties have received bad press because of the injuries from pyrotechnics and other exploding blue or pink streamers or images. But maybe beyond the danger from these explosions, the gender reveal also pigeon holes children into specific color patterns.

Don’t we want our children to think for themselves, even about their favorite colors, instead of limiting them to blue for boys or pink for girls?

Maybe, instead of colors or gender, we can celebrate the sounds of their heartbeat, the Alien-like moments when we can see their feet in their mom’s abdomen, or the foods their mothers crave during pregnancy.

The idea was that if European nations were interdependent for their economic welfare, then they would not make war on each other, but would rather work together for their greater good. And for more than 70 years, the concept held. Where wars were the way for nations, and before there were nations, for regions to enrich themselves by raiding their neighbors, stealing their treasures and claiming their land, now that was eschewed. Finally, there was to be peace.

England and France, France and Germany, Spain and England among others, all put their guns and their history away and did business with each other. This was the vision articulated by the United Nations after World War II ended, and it came to pass. The economists and philosophers were right. No one would make war on neighbors who were making them money. And for the most part, nations realized unprecedented wealth and the security that peace brings.  Economics was to be the field of battle, not the military. And with unrestricted trade, globalization took hold. War was a distant memory.

Until now. Incredible as it seemed to the rest of the world, Russia invaded the Ukraine less than two weeks ago with the aim of annexing that country. Such action, as Russian military surrounded Ukraine on three sides, would be an ill-conceived throwback to a more appalling and unwise time. Or so we thought.

As the Ukrainians defiantly rise to meet the invaders with military weapons, the rest of Europe and countries elsewhere in the world are responding with their weapon of choice: economics. It is a testament to the thinking and planning of those leaders seven decades ago. And so, with remarkable unity, the European Union is striving to blow up Russia’s economy rather than blowing up Russia’s cities. The pain for the Russian leaders and the Russian people is to be felt in their pocketbooks and not in their cemeteries. At least, that is the intent.

But of course, as in every war, it’s the civilians who most suffer and pay the price for their leaders’ actions. If they aren’t shot to death, they may be starved to death, as their money becomes worthless and their businesses are ruined. Still, the Russians will do better without Coca-Cola than the Ukrainians without water.

And that is another remarkable consequence of attempts to isolate Russia. Not only are governments withdrawing trade and financial dealings in this siege, but also international corporations are cutting ties with the invading country, even if the companies bear the price. McDonald’s, which employs some 62,000 workers in Russia, Starbucks and Apple have closed their stores, among numerous others. Americans have indicated overwhelmingly in a recent Quinnipiac University national poll (71%), that they will tolerate the increased price of gasoline if Russian imports of oil and gas are ended. The Biden administration has heard them and is closing off those imports. Of course, the prices at the pump were going up anyway due to considerable current inflation. Why not put the blame on the Russians!

So do shared economic interests prevent wars?

There should have been a corollary put into that concept: assuming all the governments are made up of reasonable persons. Much now is being made of President Vladimir Putin’s mental state because most of the rest of the world cannot understand why he is embracing this “special military operation.”

He did not even tell his lower rank soldiers that they were about to engage in a war. Who knows how the Russian leader thinks? Is he unreasonable or is this merely the opening salvo he, and perhaps his “friend,” Premier Xi Jinping of China, are plotting for a long game?

Of one thing the world can be certain. When autocrats are planning something that surely would be roundly condemned, one of the actions they take is to close down the media and crack down on free speech. Signing a new censorship law, Putin has now criminalized independent journalism for reporting “fake news.”
