Just about everyone knows the throbbing discomfort of a headache, whether it comes after a long day of work, too little sleep or an oncoming cold. It’s also likely that you’ve heard someone say they have a migraine when the pain becomes severe.

But the truth is that migraine is more than just a bad headache, and the term has taken on a variety of meanings, not all of them accurate.

According to the American Migraine Foundation, migraine is an incurable brain disease that affects approximately 40 million people in the United States — that’s 1 in 4 households. In the majority of those cases, at least one close relative has migraines as well, but it’s still uncertain what causes the disease.

Migraine can come with a wide range of neurological symptoms that differ from person to person and day to day. These symptoms exist on a spectrum from sporadic to chronic, mild to incapacitating, and some people can even experience trouble speaking, weakness and numbness in ways that mimic a stroke.

“Migraine is more than just pain. While the pain is often moderate to severe, one sided and throbbing, there are other characteristics,” said headache specialist Dr. Noah Rosen, director of the Northwell Headache Center in Great Neck. 

“The individual must also have either sensitivity to light and noise or nausea to meet the full definition. This can worsen with movement, and many people also develop associated skin or hair sensitivity. Many people may also experience changes in mood, energy level and appetite. About 20% of migraine patients may also have aura with their migraines, which is a brief, fully reversible neurological deficit. Auras can cause visual changes, sensation changes and sometimes weakness.”

For Cat Charrett-Dykes, migraines have been a regular part of her life since she was 13 years old. She would see sparkles and spots and go through bouts of nausea and vomiting, all while feeling like a knife was stabbing through her head. At school, she had trouble reading and finding the right words. “I felt like Dorothy in ‘The Wizard of Oz.’ Some of my siblings also had migraine occasionally, but not to the same degree,” said Charrett-Dykes, who lives in Holtsville. 

The attacks were relatively easy to tame until after the birth of her first child. Then, as is common, her migraines became more severe and frequent. She saw countless healthcare providers, who couldn’t agree on a diagnosis: They suggested she had anxiety, allergies, epilepsy. One even asked if her ponytail was too tight.

Unfortunately, getting a proper diagnosis and care can be a problem in the migraine community. The World Health Organization reports that more than half of all people with migraine haven’t seen a doctor for their condition in at least a year. Many more have never been formally diagnosed. While seeing a neurologist can be useful, not all neurologists are experts in headache disorders.

“Only about 700 people in the country are certified headache specialists, and the field of headache medicine is not yet formally recognized by the federal government, so there are limits on the field’s growth despite how common the condition is,” Rosen explained. “During my time as a resident physician I was seeing severely disabled patients with headache disorders end up in the emergency room, yet I had almost no education in that area, in part because of how underserved the condition is. It is often ignored, stigmatized and mistreated.”

Charrett-Dykes waited decades to find someone who understood her. 

“It wasn’t until 2003 that I was finally diagnosed. As soon as the physician’s assistant walked into the room, he took one look at me and turned off the lights,” she recalled. “No one had ever done that before. He said, ‘You have migraines, don’t you? I know that face. My wife has migraines, too.’ It was such a relief.”

Still, a diagnosis is only the beginning of the migraine journey. Treatment is focused on identifying the person’s unique triggers — perhaps certain foods, scents, strenuous activity, or an irregular schedule — along with the precise combination of medications and other options to help ease their symptoms. There is no magic bullet, and finding treatment that helps can be challenging. 

“Trigger identification and avoidance is a great thing to try, but not always possible.  Raising the ‘threshold’ required to set off a migraine can be done with pharmacological or non-pharmacological approaches,” Rosen said. “Of the medications that are available now, some are preventive and some are acute (or abortive). The preventive treatments help avoid getting the headache in the first place. Healthy habits like regulating sleep, diet, hydration and stress can reduce frequency, as can some vitamin supplements, complementary practices like acupuncture, biofeedback, mindfulness and regular cardiovascular exercise.”

The process of trial and error is exhausting for many people with migraine, including Nancy Harris-Bonk of Albany, who’s tried countless doctors and medications since her first migraine attack as a young teen. At one point, she was taking the highest dose of oxycodone allowed under a doctor’s care and still having 25 or more migraine days each month.

“I just wasn’t recovering, so I went online and started looking for answers,” said Bonk, whose episodic migraines turned chronic after a fall left her with a traumatic brain injury. “I was able to make contact with someone else who had migraine attacks, and it opened a door for me. I learned that I wasn’t alone and that there were treatment options. It made me want to help educate others about migraine disease and how to live with it.”

Downstate, Charrett-Dykes had similar goals. She founded Chronic Migraine Awareness, Inc. (CMA) in 2009, a simple chat group that later grew into a multifaceted nonprofit connecting people with resources, specialists, and one another. CMA’s main Facebook group now has 12,000 members around the world, with several smaller groups for specific demographics and topics. They also provide care packages for people with migraine, support caregivers, and lead advocacy efforts.

Bonk eventually qualified for Social Security Disability Insurance, freeing her up to focus on her well-being while acting as a resource for others. She still has about 15 migraine days a month, but medication changes and a knowledgeable healthcare team have made life a lot more manageable, she said. She serves on the board of CMA and works with the National Headache Foundation’s Patient Leadership Council; the Coalition for Headache and Migraine Patients (CHAMP); and Migraine.com.

“Learning all you can about migraine disease, knowing what it is and what it isn’t, can make a big difference when it comes to seeking care and advocating for yourself,” Bonk said. “Forming connections with others who have similar experiences is important so we know we’re not alone. This disease can leave us feeling isolated, frustrated and overwhelmed … talking with others who are going through a similar journey is validating and a great comfort. ”

While each of these organizations has a unique focus, they all share a desire to increase knowledge and awareness of migraine disease.

“The pain of migraine is not like other pain and should not be treated like that. It needs to be discussed and not just treated,” Rosen said. “The stigma of people with migraine having a low pain tolerance is also nonsense. I have been impressed on a daily basis by the strength, resilience and resourcefulness of these patients.”

To take care of your heart, it’s important to know and track your blood pressure. Millions of Americans have high blood pressure, also called hypertension, but many don’t realize it or aren’t keeping it at a healthy level.

For most adults, healthy blood pressure is 120/80 millimeters of mercury or less. Blood pressure consistently above 130/80 millimeters of mercury increases your risk for heart disease, kidney disease, eye damage, dementia and stroke. Your doctor might recommend lowering your blood pressure if it’s between 120/80 and 130/80 and you have other risk factors for heart or blood vessel disease.

High blood pressure is often “silent,” meaning it doesn’t usually cause symptoms but can damage your body, especially your heart over time. Having poor heart health also increases the risk of severe illness from COVID-19. While you can’t control everything that increases your risk for high blood pressure – it runs in families, often increases with age and varies by race and ethnicity – there are things you can do. Consider these tips from experts with the National Heart, Lung, and Blood Institute’s (NHLBI) The Heart Truth program: 

#1: KNOW YOUR NUMBERS

Everyone ages 3 and older should get their blood pressure checked by a health care provider at least once a year. Expert advice: 30 minutes before your test, don’t exercise, drink caffeine or smoke cigarettes. Right before, go to the bathroom. During the test, rest your arm on a table at the level of your heart and put your feet flat on the floor. Relax and don’t talk.

#2: EAT HEALTHY

Follow a heart-healthy eating plan, such as NHLBI’s Dietary Approaches to Stop Hypertension (DASH). For example, use herbs for flavor instead of salt and add one fruit or vegetable to every meal.

#3: MOVE MORE

Get at least 2 1/2 hours of physical activity each week to help lower and control blood pressure. To ensure you’re reducing your sitting throughout the day and getting active, try breaking your activity up. Do 10 minutes of exercise, three times a day or one 30-minute session on five separate days each week. Any amount of physical activity is better than none and all activity counts.

#4: HAVE A HEALTHY PREGNANCY

High blood pressure during pregnancy can harm the mother and baby. It also increases a woman’s risk of having high blood pressure later in life. Talk to your health care provider about high blood pressure. Ask if your blood pressure is normal and track it during and after pregnancy. If you’re planning to become pregnant, start monitoring it now.

#5: MANAGE STRESS

Stress can increase your blood pressure and make your body store more fat. Reduce stress with meditation, relaxing activities or support from a counselor or online group.

#6: STOP SMOKING 

The chemicals in tobacco smoke can harm your heart and blood vessels. Seek out resources, such as smoke free hotlines and text message programs,  that offer free support and information.

#7: AIM FOR A HEALTHY WEIGHT

If you’re overweight, losing just 3-5% of your weight can improve blood pressure. If you weigh 200 pounds, that’s a loss of 6 to 10 pounds. To lose weight, ask a friend or family member for help or to join a weight loss program with you. Social support can help keep you motivated.

#8: WORK WITH YOUR DOCTOR

Get help setting your target blood pressure. Write down your numbers every time you get your blood pressure checked. Ask if you should monitor your blood pressure from home. Take all prescribed medications as directed and keep up your healthy lifestyle. If seeing a doctor worries you, ask to have your blood pressure taken more than once during a visit to get an accurate reading.

The 2021 Atlantic hurricane season officially began on June 1, and will end on November 30. Suffolk County Legislator Nick Caracappa would like to encourage residents to be prepared. “Most of this past year’s focus has been on the Coronavirus pandemic, but we must still remember that the hurricane season is upon us.  It is important to be aware and prepared,” stated Legislator Caracappa. “Let’s all be prepared for the worst, and hope for the best.”

Know your Hurricane Risk

Hurricanes are not just a coastal problem. Find out how rain, wind, water, even tornadoes could happen far inland from where a hurricane or tropical storm makes landfall. Start preparing now.

Make an Emergency Plan

Make sure everyone in your household knows and understands your hurricane plan. In your hurricane plans include the office, kids’ daycare, and anywhere you frequent. Ensure your business has a continuity plan to continue operating when disaster strikes.

Discuss the latest Centers for Disease Control (CDC) guidance on Coronavirus (COVID-19) and how it may affect your hurricane planning.

Know your Evacuation Zone

You may have to evacuate quickly due to a hurricane if you live in an evacuation zone. Learn your evacuation routes, practice with household, pets, and identify where you will stay.

  • Follow the instructions from local emergency managers, who work closely with state, local, tribal, and territorial agencies and partners. They will provide the latest recommendations based on the threat to your community and appropriate safety measures.

Recognize Warnings and Alerts

Have several ways to receive alerts. Download the FEMA app and receive real-time alerts from the National Weather Service for up to five locations nationwide. Sign up for community alerts in your area and be aware of the Emergency Alert System (EAS) and Wireless Emergency Alert (WEA)- which requires no-sign up.

Those with Disabilities

If you or anyone in your household is an individual with a disability identify if you may need additional help during an emergency.

Review Important Documents

Make sure your insurance policies and personal documents like ID are up to date. Make copies and keep them in a secure password protected digital space.

Strengthen your Home

De-clutter drains and gutters, bring in outside furniture; consider hurricane shutters.

Get Tech Ready

Keep your cell phone charged when you know a hurricane is in the forecast and purchase backup charging devices to power electronics.

Help your Neighborhood

Check with neighbors, senior adults, or those who may need additional help securing hurricane plans to see how you can be of assistance to others

Gather Supplies

Have enough supplies for your household, include medication, disinfectant supplies, masks, and pet supplies in your go bag or car trunk. After a hurricane, you may not have access to these supplies for days or even weeks.

  • Remember that not everyone can afford to respond by stocking up on necessities. For those who can afford it, making essential purchases and slowly building up supplies in advance will allow for longer time periods between shopping trips. This helps to protect those who are unable to procure essentials beforehand and must shop more frequently. Only take the items you and your family may need so that others who rely on these products can also access them.

The local American Legion chapter has a lot going on this month.

The Wilson Ritch Post No. 432, founded in 1919, is asking Suffolk County veterans in the Port Jefferson and Port Jefferson Station area to join in their comradeship and dedication to their country’s heritage, history and constitution. 

Earlier this week on Memorial Day, the members payed homage to fallen comrades at the Crowley Park in Port Jefferson Station and Port Jefferson’s Harborfront Park. The event was followed by a dedication of land to the post from the Prechtl family, and a raffle that has earned $1,000 for two scholarships of graduating Comsewogue seniors.  

On Saturday, June 5, they will be hosting The Legion’s Convention of 46 county posts. Elected officials from the village, town and county are expected to speak.

List includes properties in Sag Harbor, Port Jefferson, Huntington, St. James, East Hampton, Southampton and East Hills.

 Preservation Long Island’s Endangered Historic Places List for 2021 includes seven sites and one district facing a variety of threats from demolition and overdevelopment, to a lack of municipal funding and support for historic preservation in communities throughout our region.

Important historic places across Long Island are threatened by a variety of adverse conditions, from outright demolition to a lack of appreciation for their historic value, or the inability to develop sustainable long-term plans for preservation and stewardship.  Preservation Long Island (formerly the Society for the Preservation of Long Island Antiquities), established the Endangered Historic Places Program (EHPP) with the goal of raising region-wide support for historic places facing precarious circumstances.

“The program offers Long Islanders an opportunity to advocate for preservation in their communities while learning how to use tools like landmark designation, tax incentives, and public outreach,” said Alexandra Wolfe, Preservation Long Island’s Executive Director. “Our program partners receive priority technical assistance from our professional staff and their listings are featured on our website and social media”.

The pandemic did not have a negative impact on the public response to the open call for nominations issued last November. “This year marks the largest number of nominations received in a single program year since the establishment of the program in 2010,” said Sarah Kautz, Preservation Long Island’s Director of Preservation and Advocacy. A total of 18 nominations were submitted for the 2021 program year from communities across Long Island.

A panel of Preservation Long Island staff and trustees, as well as experts in architecture, historic preservation, and other related fields selected the properties based on three key criteria: overall historic significanceseverity of the threat and impact the EHPP listing will have on efforts to protect the nominated site.

Each of the eight sites will be showcased in a series of free virtual programs that will include interviews and panel discussions open to the public in June and July. James Brooks & Charlotte Park Home & Studios, Springs, East Hampton Town Community Preservation Fund. Purchased by East Hampton Town in 2014, this landmarked home and studios of Abstract Expressionist artists in Springs is threatened by vandalism, disrepair, and demolition.

John Mackay III’s “Happy House”, Village of East Hills, Nassau County. One of the few remaining Gilded Age country houses in East Hills Village is threatened by demolition and subdivision.

Peter Crippen Home Site, Town of Huntington, Suffolk County. A rapidly deteriorating property owned by Huntington Town with strong ties to Long Island’s early colonial development and African American history.

Pyrrhus Concer Home Site, Village of Southampton, Town of Southampton Community Preservation Fund. Purchased by Southampton Town in 2015, restoration of this nationally significant site of African American history in Southampton Village is threatened by inter-municipal delays and other issues.

Rogers/Remz Grain & Feed Building, Port Jefferson Station, Town of Brookhaven, Suffolk County. This early industrial farming structure at the heart of Port Jefferson Station is threatened by demolition and urban renewal.

Sag Harbor Hills, Azurest, Ninevah, Subdivisions (SANS) Historic District, Village of Sag Harbor, Suffolk County. Demolition and intensive redevelopment threaten one of Long Island’s most significant Jim Crow- and Civil Rights-era historic districts in the Village of Sag Harbor.

Saint James Firehouse, Town of Smithtown, Suffolk County. A historic firehouse in active service since 1925 threatened by a lack of resources and municipal support for much-needed repairs.

William Tooker House, Village of Port Jefferson, Suffolk County. A rare pre-1750 structure in Port Jefferson Station with strong ties to Long Island’s colonial past threatened by neglect, demolition, and urban renewal.

On May 19, the Cold Spring Harbor Jr./Sr. High Locks of Love Club held their annual event for the 18th year. Participants donated their hair “safely outside” this year at The Look Spa (on Gate Street in Greenlawn) for their Locks of Love haircut, spearheaded by club advisor, Ms. Kathleen Fristensky.

It takes 15 ten-inch pony tails to create just one hairpiece for a child suffering from long term hair loss due to alopecia areata or cancer.  The children receive hair prostheses free of charge, or on a sliding scale, based on financial need. Locks of Love is a non-profit organization that provides hairpieces to financially disadvantaged children under 18 in the US and Canada.

Advisor, Ms. Fristensky, has donated her hair eight times to date!  A debt of thanks went out to Doreen Guarneri, owner of The Look Spa, who opened her doors to CSH Jr./Sr. High, thrilled to participate in this important cause.  Francesco  Martiniello, of Island Silkscreen, graciously designed and donated all the tee shirts as he does every year. he entire club continues to fundraise and bring awareness to this organization each year. CVS Pharmacy

People might soon be able to grab their prescriptions, a bag of potato chips and pick up soap as they head into a new therapy service at their local CVS.

The nation’s pharmacy retail giant has started a new service at several of their stores across the U.S. — offering both walk-in and scheduled sessions for mental health treatment. These appointments are available beyond typical daytime hours, including nights and weekends.

According to a spokesperson with CVS Health, the company began adding licensed clinical social workers trained in cognitive behavioral therapy to 13 locations in the Houston, Philadelphia and Tampa metro areas in January of this year.

These providers will offer mental health assessments, referrals and personalized counseling either in person or via telehealth through MinuteClinics inside CVS HealthHUB store locations.

The spokesperson said the company is planning on expanding to 34 locations this spring. 

“We anticipate expanding mental health services into additional markets in the coming months,” the representative said in an email. “We expect to have more detailed information soon.”

Rite Aid and Walmart have started similar programs, too.

And although MinuteClinics and the like are not available in New York state yet, Dr. Adam Gonzalez, director of behavioral health and founding director of the Mind-Body Clinical Research Center at the Stony Brook Renaissance School of Medicine, said this could potentially be a good day if such facilities were to come close to home. 

“The COVID-19 pandemic has brought about many challenges and disruptions to our way of life,” he said. “We have seen an increase in depression, anxiety, increased stress overall and grief responses. Recent mental health data from the CDC also highlights an increase in mental health problems, especially among young adults, and importantly, unmet mental health needs.”

According to Gonzalez, a recent CDC report indicates that during August 2020 through this February “the percentage of adults with recent symptoms of an anxiety or a depressive disorder increased from 36.4% to 41.5%, and the percentage of those reporting an unmet mental health care need increased from 9.2% to 11.7%. Increases were largest among adults aged 18–29 years and those with less than a high school education.” 

“There is a great need for mental health awareness and treatment at this time,” Gonzalez added. “A silver lining of the pandemic has been the expansion and use of telehealth as a platform for treatment, allowing patients to participate in mental health treatment from the comfort of their home. However, a limiting factor for accessing care is the availability of providers.”

He said throughout the pandemic, SBU’s Department of Psychiatry and Behavioral Health saw a great increase in the need for mental health care — so large, it developed a call center.

“Throughout the pandemic we have consistently been receiving over 7,000 calls per month,” the director said. “Evaluating our outpatient data for the three months before the pandemic and the current last three months, there has been a 50% increase in number of visits scheduled for outpatient mental health services. Of note, the number of visits scheduled is limited by the number of providers available to provide services.”

With the need for mental health providers continuing to grow, Gonzalez said additional services — like the ones at the CVS pharmacy chain — could be helpful.

“I think having services available at CVS could help with normalizing mental health treatment and increasing access to care,” he said. “The more mental health service options available to the community, the better. One consideration is the type and quality of care to be delivered. Although it is important to increase access, we also want to make sure that the quality of care being delivered is high.”

But if and when the retailers open up shop on Long Island, Stony Brook Medicine is working on finding ways to reduce feelings of hopelessness and distress. 

“We are actively working on investigating various methods of delivering care to help with increasing access to treatment and addressing the mental health needs of the community,” Gonzalez said. 

These methods include expanding group-based services, utilizing a collaborative care model that involves working closely with primary care physicians and behavioral health care managers for medication management, and utilizing skills-based single-session interventions. 

“We have been working with Dr. Jessica Schleider in the Department of Psychology at Stony Brook University to implement and study a problem-solving single-session intervention,” he added. “Initial data indicates that these sessions help with improving agency — motivation for change — and reduce feelings of hopelessness and distress.”
