The weather is changing and I’m getting more itchy patients coming to my office. Owners describe scratching and rashes appearing seemingly out of nowhere and they want relief for their pets. I explain to these owners that their pet most likely suffers from atopic dermatitis. 

Now, before I discuss treatment options for atopic dermatitis, we need to clearly define what it is. Atopic dermatitis in pets is defined as “a chronic, itchy, inflammatory skin condition that occurs in genetically predisposed animals.” 

What triggers this chronic condition? Pollens, mold spores, dander, dust mites, etc. Basically, atopic dermatitis equals seasonal allergies in pets. There are more choices in treatment these days and I would like to briefly go through the options.

Omega-3 Fatty Acids:  Omega-3 fatty acids are actually classified as supplements. However, they can be effective in controlling less severe cases of atopic dermatitis, or in conjunction with actual medications. The source of the highest levels are cold water fish so fish oil capsules or fish-based diets are recommended. There are also topical formulations of fatty acids that are applied directly to the skin. These oils prevent pro-inflammatory chemicals the body produces such as prostaglandins and leukotrienes. 

Omega-3 fatty acids are very safe because they are supplements but, in my experience, are rarely effective alone in treating atopic dermatitis. They work better in conjunction with some form of medication. 

Antihistamines: Histamine is a chemical produced by the body which, in small amounts, has a beneficial effect on the body. Histamine plays a role in normal cell metabolism. However, atopic patients produce larger amounts of histamine and larger concentrations of histamine in the body causes inflammation and terrible itching. Antihistamines block the histamine receptors on cells. These medications are very safe, readily available over the counter, and inexpensive. Unfortunately, they are in my opinion, very variable in their effectiveness and the least effective compared to the other choices.

Corticosteroids (cortisone derivatives): Corticosteroids include medications like dexamethasone, betamethasone, prednisone, isoflupredone, etc. These corticosteroids of different names vary in strength and half-life, or how long it takes to clear the system. These are very powerful medications and this means two things: 1) they are the most consistently effective medications in controlling the itch of allergies, and 2) they have a lot of side effects. 

The most common benign side effect is drinking and urinating more. Increased appetite and panting are also common. However, when corticosteroids are used short term for what are considered “flare ups,” the medication is generally considered quite safe. However, when used long term even at low doses, the detriment of adverse effects outweighs the benefit of the medications. These adverse effects include diabetes, pancreatitis, a suppressed immune system, stomach and intestinal ulcers, organ dysfunction, etc (this is not a complete list). 

Corticosteroids do have their place in treating what are called “flares” or acute, severe dermatitis short term in conjunction with a safer, long term treatment.

In my next article I will discuss some of the newer options used in the treatment of atopic dermatitis. As always, consult with your own veterinarian before choosing any treatments or medications.

THE FACTS: 

My mother recently passed away. She had a will which named me as executor and provided that I was to get the bulk of her estate. The balance was to be divided between by two siblings. I found a copy of her will in a file with her other important papers but the original was not in the file. I believe my brother may have the original will since he was a signatory on her safe deposit box which he emptied before my mother died. My brother and I do not get along and he refuses to take my calls.

THE QUESTION:

Is there a way I can compel my brother to turn over the will so that I can petition the court for letters testamentary and handle the probate of my mother’s estate in accordance with her wishes?

THE ANSWER: 

It is unfortunate when there is no cooperation between family members when a loved one dies but it is not uncommon. If you have asked your brother about the will and he refuses to turn it over, file it with the court or provide any information about its whereabouts; all is not lost. You can ask the Surrogate’s Court in the county where you mother lived for an order compelling your brother to provide information about the will and to turn it over if he does, in fact, have custody of the document. 

You should have an experienced attorney prepare and file with the court a petition to compel production of a will. The court will need the name and address of the brother you believe has the will, as well as the names and addresses of all other interested parties. The court also requires an original death certificate and a proposed order. 

Once the Surrogate signs the order, a certified copy of the order directing your brother to participate in an inquiry about the whereabouts of the will and to turn over to the court the original will must be served upon your brother. Your attorney then has the authority to question your brother about the will. If he refuses to cooperate with your attorney, your brother has a date certain set by the court to either turn over the will or explain why he cannot do so. The court can then decide how best to proceed. 

Although your brother’s inheritance may be smaller if your mother’s will is probated than if an administrator is appointed, hopefully your brother will do the right thing when faced with a legally enforceable court order.

When Jay Gardiner decided last year not to run for reelection for fire commissioner in the Setauket Fire District, the former chairman of the board put the cap on decades of fire rescue experience.

The longtime member of the Setauket Fire Department, who completed 30 years of volunteer service at the end of 2019, said he and his wife, Diane, are planning to move to Florida at the end of the year. While the soon-to-be 70-year-old is looking forward to spending more time with his wife and playing golf, he said he will still run his business Gardiner Plastics from a home office. The former commissioner said if he didn’t do some kind of work, he would be bored.

“I’ve been a workaholic most of my life,” he said.

When Gardiner joined the fire department in 1989, he already had 20 years of community service under his belt. Originally from Fresh Meadows, he served as an EMT in Queens with several volunteer ambulance crews.

“I’m a city boy, but I moved here in ’86,” he said. “It still makes me an old-timer, but that’s late compared to a lot of people that I know.”

Through the decades, in addition to volunteering with the department, he was an EMS lieutenant for the last 12 years. He was also an associate professor of Emergency Medical Care at Suffolk County Community College for 20 years, and he taught instructor-level courses for Suffolk EMS. At St. Francis Hospital, he was on the training faculty where he taught advance life support courses to its medical staff.

The South Setauket resident was appointed as commissioner in May of 2015 to fill the remainder of the term previously held by Thomas Gallagher. Later that year, Gardiner was elected to a five-year term as commissioner, and during his last three years on the board, he served as chairman.

When Gardiner first ran for fire commissioner in 2015, he said he felt his business background would come in handy. The former commissioner holds an MBA from the NYU Stern School of Business and has served on several organizational boards in the plastic industry.

When it comes to the board of fire commissioners and the departments in the district, Gardiner sees it as a team effort, and he’s proud of what the district has accomplished over the last few years.

“You can’t get the fire truck or the ambulance to the scene with one person,” he said. “It takes a group of people. It takes an officer. It takes a crew. It takes a driver. And if you don’t have all those working together, you don’t have an efficient department or an efficient board. The board has to work together, we have to be a team, and we’ve been very fortunate.”

Some of the accomplishments Gardiner listed include the renovation of the main firehouse on Route 25A, the purchase of four new fire engines and updated equipment such as radios and air packs to modernize the emergency system. He also counts the addition of a few career firefighters during the day to the department as an accomplishment of the board.

“We were the first one on Long Island to actually call them career firefighters,” Gardiner said. “Nobody wanted to take that jump. It’s not an indictment on the volunteers.”

While it was a tough decision, he said the commissioners have a responsibility to the community. 

“Your job is a fiduciary responsibility to take care of that community, and 30 years ago, there were loads of people who lived in the community during the day, they could respond to the alarms,” he said. “The demographic has totally changed.”

With his departure from the board of commissioners, one of his teammates for decades, John Wastiewiz, took over as chairman of the board. Wastiewiz said he has known Gardiner since before the latter joined the fire department, when his wife worked for Gardiner Plastics. Wastiewiz described the former commissioner as the ultimate professional, and said he considered Gardiner the go-to guy, especially when it came to questions about EMS.

“Everything he’s done, whether it’s paramedic or fire commissioner, he’s always strived to be the best and constantly improving his skills and education, and he’s just a very good guy in general.” Wastiewiz said.

Gardiner said when it comes to being a volunteer firefighter he misses responding
to scenes.

“My wife always looks at me and she goes, ‘You miss it, don’t you?’” he said. “I say, ‘Yup,’ but everything has its time.”

When he moves to Florida, he said he will also miss the Three Village area, where his children grew up and went through the Three Village school district. He said he will especially miss the sense of local history and the area near Emma Clark library and the Frank Melville Memorial Park.

“We’ve always loved living in this area,” he said. “It has good restaurants and good people. The civics and the chamber of commerce do a lot of work to try to build up the area.”

However, while Gardiner and his wife may be moving, he said they will be back to visit as their four children, three daughters-in-law and three grandchildren live on Long Island.

When asked if he had some advice to share with his fellow Setauket firefighters, Gardiner said it’s important to remember the constant commitment to the community. 

“Never get complacent,” he said. “There’s always room for improvement. It gives you the motivation to push forward. You don’t want to say, ‘I’m done.’”

Type 2 diabetes is pervasive throughout the population, affecting adults, but also children and adolescents. Yet, even with its prevalence, many myths persist about managing diabetes.

Among these are: Fruit should be limited or avoided; Soy has detrimental effects with diabetes; Plant fiber provides too many carbohydrates; and Bariatric surgery is an alternative to lifestyle changes.

All of these statements are false. My goal is to help debunk these type 2 diabetes myths. Let’s look at the evidence.

Fruit

Fruit, whether whole fruit, fruit juice or dried fruit, has been long considered taboo for those with diabetes. This is only partially true. Yes, fruit juice and dried fruit should be avoided, because they do raise or spike glucose (sugar) levels. The same does not hold true for whole fresh or frozen fruit. Studies have demonstrated that patients with diabetes don’t experience a spike in sugar levels whether they limit the number of fruits consumed or have an abundance of fruit (1). In another study, whole fruit actually was shown to reduce the risk of type 2 diabetes (2).

In yet another study, researchers looked at the impacts of different types of whole fruits on glucose levels. They found that berries reduced glucose levels the most, but even bananas and grapes reduced these levels (3). That’s right, bananas and grapes, two fruits people associate with spiking sugar levels and increasing carbohydrate load. The only fruit that seemed to have a mildly negative impact on sugars was cantaloupe.

Whole fruit is not synonymous with sugar. One of the reasons for the beneficial effect is the fruits’ flavonoids, or plant micronutrients, but another is the fiber.

Fiber

We know fiber is important for reducing risk for a host of diseases and for managing their outcomes, and it is not any different for diabetes. 

In the Nurses’ Health Study (NHS) and NHS II, two very large prospective observational studies, plant fiber was shown to help reduce the risk of type 2 diabetes (4). Researchers looked at lignans, a type of plant fiber, specifically examining the metabolites enterodiol and enterolactone. They found that patients with type 2 diabetes have substantially lower levels of these metabolites in their urine, compared to the control group without diabetes. There was a linear, or direct, relationship between the amount of metabolites and the reduction in risk for diabetes. The authors encourage patients to eat more of a plant-based diet to get this benefit.

Foods with lignans include flaxseed; sesame seeds; cruciferous vegetables, such as broccoli and cauliflower; and an assortment of fruits and whole grains (5). The researchers could not determine which plants contributed the greatest benefit. The researchers believe the effect results from antioxidant activity.

Soy and kidney function

In diabetes patients with nephropathy (kidney damage or disease), soy consumption showed improvements in kidney function (6). There were significant reductions in urinary creatinine levels and reductions of proteinuria (protein in the urine), both signs that the kidneys are beginning to function better.

This was a small randomized control trial over a four-year period with 41 participants. The control group’s diet consisted of 70 percent animal protein and 30 percent vegetable protein, while the treatment group’s diet consisted of 35 percent animal protein, 35 percent textured soy protein and 30 percent vegetable protein.

This is very important since diabetes patients are 20 to 40 times more likely to develop nephropathy than those without diabetes (7). It appears that soy protein may put substantially less stress on the kidneys than animal protein. However, those who have hypothyroidism should be cautious or avoid soy since it may suppress thyroid functioning.

Bariatric surgery

Bariatric surgery has grown in prevalence for treating severely obese (BMI>35 kg/m²) and obese (BMI >30 kg/m²) diabetes patients. In a meta-analysis of bariatric surgery involving 16 randomized control trials and observational studies, the procedure illustrated better results than conventional medicines over a 17-month follow-up period in treating HbA1C (three-month blood glucose measure), fasting blood glucose and weight loss (8). During this time period, 72 percent of those patients treated with bariatric surgery went into diabetes remission and had significant weight loss.

However, after 10 years without proper management involving lifestyle changes, only 36 percent remained in remission with diabetes, and a significant number regained weight. Thus, whether one chooses bariatric surgery or not, altering diet and exercise are critical to maintaining long-term benefits.

There is still a lot to be learned with diabetes, but our understanding of how to manage lifestyle modifications, specifically diet, is becoming clearer. The take-home message is: focus on a plant-based diet focused on fruits, vegetables, beans and legumes. And if you choose a medical approach, bariatric surgery is a viable option, but don’t forget that you need to make significant lifestyle changes to accompany the surgery in order to sustain its benefits.

Stony Brook University Hospital has taken a step in offering cardiac diagnosis and treatment that is even more advanced than in the past.

Recently, the hospital announced the opening of its Cardiac Catheterization and Electrophysiology Advanced Multifunctional Laboratory in the Stony Brook University Heart Institute at SBUH. The lab consolidates comprehensive cardiac catheterization and electrophysiology services into one location.

The multifunctional laboratory measures 845 square feet to allow room for various medical teams to perform emergency procedures at the same time if needed. The room includes anesthesia equipment, state-of-the-art angiographic suite equipment and the latest electrophysiology technology. In the lab, physicians are able to continue treating a patient even if the scope of a procedure changes from minimally invasive to more invasive.

When it came time to design the multifunctional laboratory, administrative and medical professionals were able to provide input including Cath Lab Director, Dr. Robert Pyo and EP Lab Director Dr. Eric Rashba.

Pyo said it was important to get input not only from doctors but nurses and technicians, who play a crucial part in documenting procedures, information that will be used during a patient’s treatment.

Rashba said time was spent with the construction group to ensure everything was laid out correctly and that it would work for both specialties in the multifunctional lab. He added that work began April 12 to renovate five existing labs, three Cath and two EP, adjacent to the new Cath/EP lab on the main level of the Heart Institute. One lab at a time will be worked on, and while the additional renovations will take several months, Rashba said the number of patients that Stony Brook doctors can treat will increase, and patients will be able to get appointments quicker than in the past.

“What we’ve seen over time in electrophysiology is that you see more and more patients with arrhythmias that need treatment,” he said. “There’s been an incredible growth in ablation procedures, in particular atrial fibrillation. This will allow us to meet the community need with less waiting times for procedures. So, we’re looking forward to that.”

Since the lab opened March 30, both doctors said the imaging has been superior to what they had been using before. The lab includes an image-guided diagnostic and therapeutic imaging system called the Philips Azurion 7.

“We’re replacing systems that have been installed for over 10 years,” Rashba said. “First of all, we can see a lot better what the definition of the structures are we need to see, plus the radiation definition is a lot lower. So, we’re getting better imaging with less dose to the patient.”

Rashba added that some EP procedures can even be done without radiation.

Pyo said the new multifunctional lab also saves doctors precious time when treating heart patients with both catheterization and electrophysiology in the same room.

“The importance of timing, reducing the time to treatment, whether it’s minutes or seconds, is relative,” Pyo said. “I think that in any case, even in patients who come in electively, getting early diagnosis is crucial.”

Being able to respond quicker is especially crucial with treatment of heart attacks.

“Patients who are presenting with a heart attack, minutes, even seconds, count toward early diagnosis and treatment,” Pyo said, adding if patients don’t get treatment early enough they could suffer irreversible damage.

There’s a place for people of all abilities to play soccer, and it’s right in Centereach.

TOPSoccer is a new program within Middle Country Children’s Soccer devoted solely to children with physical, mental or behavioral disabilities.

Rob Draper, a financial advisor at Draper Asset Management in Smithtown and club president of SB Athletico Soccer Club, said he wanted to provide a place where all different types of people can run, play and score goals together without judgment or fear. 

“Soccer helped me experience my kids’ childhoods,” he said. “With soccer, there were times where I would go, ‘Wow. That’s great,’ and our goal is to give those moments to these parents so they can experience it, too.”

Draper teamed up with Dave Phaff, also of SB Athletico, and James Malone, president of Middle Country Children’s Soccer to make this dream a reality.

“We’re all just good-hearted people that just really enjoy working with children and watching them grow into young women and men,” Phaff said. 

Officially kicking off last weekend, the program is made up primarily of skills sessions that help teach the participants the game of soccer, while at the same time providing them with an environment that is fun and pressure-free.

Phaff said the whole process took less than three months to get off the ground, and everyone was immediately on board. 

Joining the team is Tara Phaff, Dave’s wife — who is also a Doctor of Physical Therapy — who helped create the best plans for each individual participant’s needs.   

According to Middle Country TOPSoccer, the team encourages any child or adult who is differently-abled to get involved — and its 100% free.

With no questions asked, Draper wanted to fund the program for the families who decide to join in.

“Rob said to me, ‘I really want to start a special needs soccer program and I’ll fully fund it. I don’t care what it costs, I’ll fund it,’” Phaff added. “He has a great heart.”

While other soccer clubs do encourage inclusivity, Phaff said there aren’t too many locations where kids of different abilities can play further out east. That’s why Centereach was a great, central spot to get the ball rolling.

On Saturday, April 10, the group held its opening day at Oxhead Road Elementary School where 25 individuals, ranging in ages five to 55, headed to the field to kick, run and enjoy the sunshine. 

Some of the participants have Down syndrome, some on the autism spectrum and two children were able to play soccer from their wheelchairs. One thing they all had in common was the giant smiles on their faces.

Each player, Phaff said, gets a buddy that works with them every practice. These buddies are volunteers and soccer players, themselves, who help the individual and stand beside them the whole time — and they are anticipating even more players to come join them this season.

“The whole purpose of this is to give these kids an opportunity to be accepted and feel like they are part of something,” Phaff said. “So, we won’t turn anybody down.”

TOPSoccer has their practices every Saturday at 10 a.m. at Oxhead Elementary School in Centereach. Families who are interested in joining can register online at mccsoccer.org.

The Three Village Chamber of Commerce welcomed Koeppel Dental Group (A Dental 365 Company), Druthers Coffee and Jersey Mike’s Subs to the community with a celebratory ribbon cutting on April 5. All three businesses are located in Stony Brook Square at 1113 North Country Road in Stony Brook. 

The event was attended by New York State Assemblyman Steve Englebright, Kim Bryant from Suffolk County Legislator Kara Hahn’s office,  Town of Brookhaven Councilman Jonathan Kornreich and members of the chamber. 

“The Three Village Chamber of Commerce is working to beat back the economic impact of COVID-19 by helping new businesses open and existing businesses safely reopen. As someone who knows just how difficult it is to start a small business, I applaud the entrepreneurs behind Druthers Coffee, Koeppel Dental Group, and Jersey Mike’s Subs for opening their doors here in Three Village,” said Councilman Kornreich in a statement. 

“Main Street America’s Small Business Day took place on April 13, and I’m optimistic that we will see more growth and opportunity across the First Council District,” he added.

The former Café Bada-Bing finally went “bada-bye.”

Steps away from the train station in Port Jefferson, construction crews began to knock down the former vacant bar on April 12.

Last known as the Bahia Bar & Discotec, the plot will soon be home to a new mixed-use site with 45 units of housing and more than 3,000 square-feet of ground floor commercial space to be called Port Jefferson Crossing.

The company behind the build, Conifer Realty, LLC, joined village officials as demolition began.

Village Mayor Margot Garant put her hard hat on and got behind the bulldozer, to help knock down its first wall. 

“This is a revitalization project that’s been going on now for many years,” she said. “We’re finally out of the planning process and into the building process.”

This is the first step toward giving Upper Port its much-needed facelift, including revitalizing the train station, building affordable and safe housing for young people and senior citizens close to the LIRR.

According to the IDA, Port Jefferson Crossing is a $24 million project that will construct 45 units of residential workforce housing in the heart of Port Jefferson.

The affordable housing component will be given out based on a lottery system, and will be located at 1609-1615 Main Street, currently the site of two vacant buildings.

“The public private partnership with the Brookhaven IDA, Suffolk County and the Village of Port Jefferson is instrumental in bringing Port Jefferson Crossing to fruition,” said Roger Pine, vice president of Development of Conifer, in a statement. “This is a long-term partnership that will bring continued revitalization to areas most in need in Long Island.” 

Garant added that this project was a collective effort of several state agencies to bring life back to uptown, and Suffolk County Executive Steve Bellone (D) complimented the mayor for her efforts. 

“Some good things are happening,” he said. “You’re making things happen here on the local level. You’re doing the things necessary to create   vibrancy, to create opportunity to create a place that people will flock to here, right around this train station.”

He said that a mixed-use building like this one will make the region more attractive to young people.

“We need to build a growing sustainable innovation economy,” he added. “You’re at the fore-front of doing that work and making it happen. And certainly, building more affordable housing in our downtown is key, not only to revitalizing and creating more vibrancy downtown, but to creating a prosperous economy.”

The land-clearing demo will take about two weeks before its ground-breaking event and the actual building.

The Sound Beach Civic Association wanted to do something for local essential workers, and everyone knows that food brings people together.  

Bea Ruberto, president of the civic, said early last year the group wanted to do a tribute to the frontline and essential workers at an adopted spot they take care of on New York Avenue.  

Although still a concept, the plan is to plant a small tree with a memorial stone in front of it to honor all the people who risked their lives. 

But due to the ongoing COVID-19 pandemic, the civic was unable to do typical fundraising to make the tribute happen. So, instead, they decided to create a cookbook, “Signature Dishes of Sound Beach and Beyond,” for donations. 

“Because the civic is a not-for-profit, we are asking for a donation of $20 and then they get the book,” Ruberto said.  

The cookbook, which came out officially earlier this month, is dedicated to all the people who kept everybody safe throughout the coronavirus crisis, and features a section devoted solely to the local heroes. 

Ruberto and her group began reaching out to civic members and others in the Sound Beach community asking them to send in their favorite family dishes.

“Initially, we wanted it to be just people in Sound Beach, but we weren’t getting enough recipes,” she said. “So, then we reached out beyond that, which included friends of people who live in Sound Beach, families of people who live in Sound Beach, and this is how it came together.” 

She added that by the end of production — which took about four months to put together — 64 people submitted 220 recipes for the 150-page book. 

Sound Beach resident Mary McKeown submitted the most recipes — 15 of them — and would reach out to Ruberto often to ask what kind of recipe she needed. 

“I just love all the people we’ve met here, and just to be a part of everybody doing it all together has just been great fun,” McKeown said.  

Brianna Florio, also of Sound Beach, volunteered to create the cover and drew the concept drawing for what the tribute at the adopted spot will look like when completed. 

“I think she did a beautiful job,” Ruberto said.  

The black-and-white cover features a local beach with a picnic basket and birds flying in the background.  

The 22-year-old said she took a photo of the Long Island Sound and then used her Photoshop skills to add the details.  

As of right now, more than 100 books have been sold to people throughout the whole country. 

“I actually I got an email from somebody who saw this on Facebook from North Carolina,” Ruberto said. “And he wanted to gift it to his mom who used to live in Sound Beach.” 

Ruberto said that the cookbook will appease all different types of palates because there is so much variety, but the classics still remain. 

On page 142, there is a recipe from two local boys for one of their favorites: pea-nut butter and jelly.
