Registered nurses at St. Catherine of Siena Medical Center in Smithtown and St. Charles Hospital in Port Jefferson have been working without a contract since March 2015, but they may be nearing a tipping point.

Nurses from both hospitals voted overwhelmingly to authorize a strike this week, according to a statement dated Oct. 17 from the New York State Nurses Association, a union that represents about 40,000 registered nurses in the state.

“We are very frustrated with management,” Tracy Kosciuk, a St. Charles nurse in the maternal child unit who has been with the hospital for nearly three decades, said in a statement. Kosciuk is also the president of the union’s executive committee for St. Charles. “We feel a total lack of respect. Our community appreciates our dedication and management should too.”

Spokespersons from both hospitals responded to the union’s actions in emailed statements.

“We are working diligently to resolve all issues and have made great progress toward that goal,” separate statements from St. Charles executive director for public and external affairs, Marilyn Fabbricante, and St. Catherine’s executive vice president and chief administrative officer, Paul Rowland, each said. “We look forward to a mutually satisfactory collective bargaining agreement which rewards our nurses and meets the needs of our hospitals.”

Fabbricante added that St. Charles has not yet been informed by the union of plans to go on strike. Carl Ginsburg, a spokesman for the union, said they have strike authorizations from members at both hospitals, though negotiations are ongoing and no dates have been determined for the strikes. Unions are required to give health care institutions at least 10 days notice prior to a strike, according to the National Labor Relations Board — and Ginsburg said that had not yet occurred.

According to the release from the union, its members are frustrated by inadequate staffing and are seeking better health benefits and a pay increase in their next contract. In 1995, performance of nurses and other health care professionals became subject to ratings based on patient surveys conducted by the Agency for Healthcare Research and Quality, according to its website.

“All of these issues affect retention and recruitment,” Tammy Miller, a nurse at St. Catherine of Siena, said in a statement. “Keeping and attracting experienced nurses are essential to quality care.”

Dawn Bailey, a registered nurse and labor bargaining unit executive committee member of the NYS Nurses Association, said during a picket outside the Smithtown medical center in April that working a shift without adequate staff can be dangerous not only for patients, but for nurses as well, on top of the potential damage it can do to ratings.

“You can’t have two people lifting a patient all the time because there’s not that other person available,” Bailey said. “When people are going out with back injuries, then [members of hospital administration] wonder why.”

Kosciuk expressed a similar sentiment during a picket in April.

“Unfortunately the mentality … nowadays in the industry is [that] all hospitals are short staffed,” she said. “That’s not acceptable to have that mind-set.”

Suffolk County Police have arrested a 24-year-old Sound Beach woman for driving while intoxicated with children in her car in Miller Place Oct. 15 at about 10 p.m.

A Good Samaritan called police after she noticed an apparently intoxicated woman putting her children in and out of her car, while illegally parked in a handicapped zone in front of Stop & Shop in Miller Place. Officers stopped the Lincoln Navigator on Carlisle Road in Miller Place at about 10:30 p.m. The woman, Holly Olsen, had two children ages two-years-old and two-months-old in her car.

Olsen was charged with driving while intoxicated, aggravated driving while intoxicated with a child passenger 15 years old or younger (Leandra’s Law), two counts of endangering the welfare of a child, and second-degree aggravated unlicensed operation of a motor vehicle.

Olsen was be held overnight at the 4th Precinct and was scheduled to be arraigned at 1st District Court in Central Islip on Oct. 16. Attorney information for Olsen was not immediately available.

Brookhaven Town is one of many municipalities being forced to tighten its belt for 2017. Faced with the unenviable task of crafting an operating budget for 2017 with very little leeway to increase revenue from taxes, the town released its tentative budget for the coming year at a board meeting Sept. 29.

The town’s tentative operating budget for 2017 stands at just under $282 million, which represents a $1.8 million increase over the adopted 2016 operating budget.

Despite a state-mandated tax levy increase cap of just 0.68 percent, a contractually mandated 1.5 percent increase in salaries and a projected increase in health benefits of more than 9 percent for town employees, the budget will maintain all town constituent services and quality of life enforcement resources, according to a statement by Supervisor Ed Romaine (R).

“The strength of this town and its finances continues thanks to the hard work of all of our [councilpersons] and to the hard work of our Finance Commissioner Tamara Wright and [Chief of Operations] Matt Miner,” Romaine said during the meeting.

Wright presented the budget to the public during the meeting, which she called the most difficult of the nine town budgets she’s worked on to date.

“As the supervisor indicated, this budget is in compliance with the New York State tax cap, which is at an unprecedented low for the 2017 fiscal year,” Wright said. “It was a challenge to meet that, but we’re very pleased we were able to stay within the cap.”

The town was granted a AAA credit rating by Standard & Poor’s Financial Services earlier this year. Romaine and Wright both said maintaining that rating played a roll in the budget process.

“We have significantly reduced the use of surplus to balance the budget, using only $3.1 million this year,” Wright said. “That is a significant reduction over our previous years and it is what Wall Street and the credit rating agencies look for to maintain as AAA. This particular budget continues to build on the principles we’ve been putting in place over the past several years that have lead to the AAA rating, and I’m confident that it is going to allow us to keep it as we move forward.”

One area that saw an increase in funds for 2017 was snow removal, which now stands at just under $6 million.

“If we do have a year of little snow, we hope to be able to move some of that budget over to highway maintenance [budget] lines to increase the spending there if we don’t need the snow budget,” Wright said.

The town was able save money by eliminating 13 full-time positions, which were left vacant primarily by retirements and resignations during the course of the year, according to Wright.

The town board will hold a public hearing and vote to approve the tentative 2017 budget Nov. 10 at 5 p.m.

The remnants of Hurricane Matthew made for a sloppy morning, but that didn’t stop more than a thousand runners who laced up their shoes and hit the streets of Port Jefferson for a good cause.

The 15K Run to the Port Jeff Brewing Company took place Oct. 9 to raise money for the Port Jefferson “Treasure Your Parks” campaign, an initiative created to help refurbish the more than 50-year-old Clifton H. Lee Memorial Park, which is commonly known as Rocketship Park.

The roughly nine-mile race began on West Broadway near Schafer’s restaurant, and concluded on Mill Creek Road near the Port Jeff Brewing Company, where participants celebrated the run with a cold beer. Runner Chris Steenkamer crossed the finish line first, and Chris Koegel came in second.

An event to kick off the refurbishing process, called the Rocketship Park Launch Off, will be held Oct. 13 at 4:30 p.m. at the park located behind Port Jefferson Village Hall about a half a block away from Port Jeff Brewing Company. For more information visit www.rebuildrocketship.org.

Medical professionals, law enforcement and government officials gathered at Stony Brook University this past weekend to have an open and honest dialogue about the growing opioid problem facing the North Shore and the rest of Suffolk County.

The complexity of the problem and how it relates to communities across the county was the topic of the discussion Oct. 1. A common theme among the speakers was opioid addiction should be treated like a legitimate medical crisis and not a moral failing.

Dr. Constantine Ioannou, director of Stony Brook Medical Center’s Adult Inpatient Unit addressed the current opioid crisis in the United States and specifically in Suffolk County during the event.

“This is not the first opioid epidemic in the United States — this is one of many,” Ioannou said. He likened the current state of opioid prescribing and subsequent widespread addiction to a period in the late 1800s when morphine was first developed. He said doctors overprescribed the powerful painkiller and, in turn, opioid dependence skyrocketed.

The director traced the origin of the current crisis back to two events in 1995. Purdue Pharma, a pharmaceutical company, began marketing OxyContin, its version of the powerful opioid oxycodone, to doctors. In addition the American Pain Society, an organization dedicated to advocating for public policies to reduce pain-related issues, named pain as the fifth vital sign. Like the other four — pulse rate, temperature, respiration rate and blood pressure —  pain would be monitored in patients from then on. Pain was the only one of those five vital signs that is completely subjective and based on what a patient tells a nurse or doctor, Ioannou said.

Those two events, in accordance with nurse ratings and even payment being tied to patient satisfaction and reduction, created an environment of overprescribing, Ioannou said. He also said training of doctors in pain management needs to be addressed — he graduated from medical school in 1985 with “zero” training in pain management.

“There are states in the United States where there are more prescriptions for opiate pain medications than there are people — this is a staggering number,” Ioannou said.

Jermaine Jones, Ph.D., an assistant professor of clinical neurobiology in the Department of Psychiatry at Columbia University, also spoke and reiterated some of what Ioannou said regarding the United States’ views on pain management.

“I’ve had friends from other countries say that unless you come into the E.R. with an arm missing, you’re not going to get a prescription that you could get here for having a root canal,” Jones said. “We make up about 5 percent of the world’s population yet we consume about 80 percent of the prescription opioid [painkillers].”

Jones is involved in a study called Risks and Benefits of Overdose Education and Naloxone Prescribing to Heroin Users and spoke about some of his findings. Naloxone, commonly known by the brand name Narcan, is a drug used to reverse opioid overdoses.

“We’re trying to better understand how to tease apart ways to attenuate what people like about opioids,” Jones said of the five-year study, which is currently in its second year.

He said the study was created to monitor people who are administered naloxone and see if it is being used for the correct purposes. One concern in the creation of an overdose reversal drug is that users will be more confident in their ability to combat an overdose and may decide not to call 911 if that day ever comes. Other potential unintended consequences of naloxone, including how to deal with a user who may be irate after being saved from an overdose and is now “dope sick” and without the expensive drug they just bought, are the focus of the study. They are also researching some potential new medications or existing ones that could work in accordance with opioids to reduce pain, while reducing the effects that lead to addiction.

“We know that detox by itself is actually one of the highest risk factors for opioid overdose because you’re tolerance decreases,” Jones said. “So once someone comes out of a detox program they swear they’re never going to touch this stuff again, but relapse is very, very common. So they use again thinking that if they were using two bags before, they can continue using two bags now that they’ve gotten out, but their physiological tolerance has decreased and people overdose as a result.”

Ioannou indicated he’d like to see changes in treatment options for addicts.

“We treat all addiction by the same model,” he said. “You have a five-day detox, 28-day rehab and you have an after care that is all based on 12 steps. That is the model of care in the United States. We need to realize with a complex disease, you need a complex set of interventions.”

Suffolk County Police Department Deputy Sheriff Mike Kern is an expert in recognizing drug users. He called overdoses the most powerful “advertisement” for drug dealers because it is a clear indication of how strong their product is. He echoed sentiments from both doctors about the dangers of overprescribing and to what it can lead.

“Why can a doctor prescribe an OxyContin to a 16-year-old or a 15-year-old who just had a root canal?” Kern asked. “It just doesn’t make any sense.”

Suffolk County has a drug problem. And while it may be broken news, this is not breaking news.

Heroin and prescription opioid-related overdoses and deaths are increasing yearly across the nation, state and county, according to all available data, but the overall conversation lacks focus, those close to the issue have said. One Long Island man whose line of work leaves him with little insulation from the problem said it is worse than most would imagine.

Dan Moloney, who along with his brother Peter owns six Long Island locations of Moloney Family Funeral Homes, said in an exclusive interview that he believes the problem facing Suffolk County deserves a harsher spotlight. Moloney, who has an unenviable front row seat to the horrors that come from the addictive and powerful substance, said the problem reached a tipping point for him in 2009.

After a funeral for a Rocky Point student who overdosed, the Moloneys decided to try to use their platform to deliver an important message. They had posters made up with the words “Some kids are dying for a high” in bold letters on top of an image of a flower arrangement with a card that read, “With Deepest Sympathy, The class of ’10.” Below the image, the funeral director’s message read in part: “The last thing we want to see is a death that could have been prevented. Help us make sure we don’t.”

The Moloneys tried to distribute the posters to school districts around the Island, though they couldn’t find any takers.

“Nobody wants to talk about it,” Dan Moloney said. “Nobody wants to hear from the funeral director.”

On the surface, in Suffolk County, it would appear heroin abuse is a daily conversation in one way or another, from politicians sponsoring initiatives to news outlets covering arrests and overdoses, to firsthand accounts from former addicts in various forms.

Moloney said he wouldn’t agree — not only is the problem receiving too little attention, he said, but also the wrong people are doing the talking.

“Are people sitting in the bleachers talking about the heroin problem?” he said. “But if their kid had some sort of disease, they’d be talking about it. They’d be doing fundraisers to help them find a cure.”

The two go hand-in-hand — heroin and opioids — or at least they should, Moloney said. Heroin is an illegal and highly addictive version of an opioid, according to the Centers for Disease Control and Prevention. However, statistics reporting deaths related to one or the other are not always paired.

The CDC’s website said health care providers wrote nearly a quarter of a billion prescriptions for legal opioids in 2013. Supply and demand for prescription pain medication doesn’t always dry up at the same rate. When the prescribed pills are gone and the desire for more lingers, the cheaper, stronger drug becomes a logical alternative.

In 2013, New York State’s Internet System for Tracking Over-Prescribing — Prescription Monitoring Program, also known as I-STOP/PMP, went into effect. The system works as a registry for practitioners to consult and track dispensed prescription histories for patients. The program has restricted supply of opioids to addicts, though it has done nothing to curb their demand. No tracking system exists for the neighborhood heroin dealer.

Moloney said one of his business’s facilities held funerals for three heroin overdose victims in just one day earlier this year. The closest comparison he could come up with to a public health concern inflicting that much damage in one day is a car crash that kills a vehicle full of people. He said that in some years, only two to three motorcycle-related deaths occur over the course of entire summer, which the public tends to find alarming, but that pales in comparison to heroin- and opioid-related deaths.

The difficulties in securing relevant and timely statistics on overdose-related deaths in New York State has contributed to undermining the understanding of the severity of Suffolk County’s problem, according to Moloney.

“New York State is terrible,” he said about the state’s demographic record-keeping, which is an insight few could offer outside of the funeral business. “Three years down the road — the latest data you have is from three years ago. With the technology we have today, there’s absolutely no reason for that. And I know from colleagues that I have in other states, when you can’t get the information about how many deaths occurred in a certain place for two or three years, or what they were — because all of that is tracked — I just think the data that’s out there is antiquated and the situation is worse than the data they’re using shows.”

Father Francis Pizzarelli, director of Hope House Ministries in Port Jefferson, has been a regular contributor of insight and opinions regarding heroin and opioid addiction among young people in Suffolk County for about as long. He, like Moloney, said the problem is likely worse than anyone in the county realizes.

“The level of denial among parents continues to be deeply disturbing,” Pizzarelli wrote in a April 2016 column featured in this newspaper. Alcoholics Anonymous and Narcotics Anonymous, “which are a vital lifeline and network in our community for those working on recovery and wellness, have to worry that drug dealers are now waiting outside these meetings to prey on men and women in early recovery.”

Pizzarelli said his tipping point, much like that of Moloney’s, came in 2009. So far, though, he added, it has not been enough.

Suffolk County Police Commissioner Tim Sini said in May there were 103 fatal heroin overdoses in Suffolk County in 2015. New York State Comptroller Tom DiNapoli (D) released a report on June 9 saying there were more than 200 deaths in which heroin or opioids played a role in Suffolk County in 2014.

Regardless of how and when the deaths are identified with a specific cause or a contributing factor being opioids, one thing is clear to Moloney: the number is higher than we think.

According to the Suffolk County Police Department, since the act of administering the medication Narcan to reverse an opioid overdose became commonplace in August 2012, more than 630 saves have been recorded through Sept. 22.

In addition to conflicting stats, Moloney said an issue that he encounters is the stigma parents feel about losing a child to an overdose and what it might suggest about their aptitude as a parent. Most of the time, parents decline to immediately identify a heroin or opioid overdose as their child’s cause of death, he said. In fact, Moloney estimated that nine out of 10 parents whose child died of an overdose don’t address the issue and the cause isn’t added to a death certificate until about three months later, when lab reports are complete.

“It almost creates an environment where there doesn’t have to be an acknowledgement —not publicly,” Moloney said. “Of course there’s a lot of shame.”

A boat overturned killing two people in the Long Island Sound just west of the Port Jefferson Channel in the afternoon Oct. 4. Suffolk County Police Homicide Squad detectives are investigating the incident.

At about 3:35 p.m., a boater alerted the Coast Guard that there was an overturned boat in the Long Island Sound. U.S. Coast Guard officials responded and recovered the body of Charles Petrie, 80, of Holtsville. Suffolk County Police Marine Bureau officers also responded and assisted the Coast Guard in searching for James Bilello, 73, of Bay Shore, who was believed to be with Petrie and was missing until his body was found at about 11:30 a.m. Oct. 5 near the lighthouse at the end of Old Field Road in Old Field.

Marine Bureau officers recovered the boat, a 20-foot AquaSport center console, which was impounded for a safety check.

A growing trend this election season amongst newspapers, politics-centric websites, pollsters and even candidates is to fact-check claims made by presidential hopefuls or their litany of staffers during speeches, debates and other public forums in real time.

In theory, that makes perfect sense. Candidates should be taken to task for false claims they make in public when attempting to appeal to voters. During the first presidential debate, Sept. 26 at Hofstra University in Hempstead, between Democratic nominee Hillary Clinton and Republican nominee Donald Trump, each took turns making statements and accusations that were later proven false by the army of fact-checkers listening closely.

Trump asserted that the stop-and-frisk policy did wonders for crime rates in New York City during its short-lived run. Fact-checks by the Associated Press, the Washington Post and CNN yielded no proof of stop-and-frisk impacting crime rates. Trump accused Clinton of “flip-flopping” her position on The Trans-Pacific Partnership, a global trade deal, which she initially supported and referred to as “the gold standard.” The same cast of fact-checking characters nabbed Clinton for switching positions in the debate aftermath.

Fact-checking during and immediately following the first presidential debate was a useful tool for American voters. However, if checking facts were this important throughout the primary process, it’s possible Americans might be choosing from a different slate of candidates Nov. 8.

Our editorial staff wonders how much of an effect fact-checking has on voters. How many Trump and Clinton supporters heard their candidate say something that was later proved false, and actually started reflecting on if that mattered to them? Fact-checking is important, and it’s great that so many media outlets are devoting resources to it. It’s part of what separates news organizations from the rest of the social media storm that ensues during and after major events. We hope the increase in fact-checking doesn’t fall on deaf-ears, and voters take notice of when their candidates are proven wrong.

Don’t judge the Port Jefferson Free Library by its cover, at least not before ground is broken on potential renovations.

The board of directors at the Port Jefferson Free Library are mulling ideas for upgrades in the hopes of meeting the needs of the community as a 21st century library. At a public meeting Sept. 14 at the library, the board and architects from Patchogue-based BBS Architects & Engineers discussed options for upgrades and listened to input from the community. In addition to its annual operating budget, the volunteer organization, Friends of the Library, is seeking donations from the public to be able to afford the total cost of potential renovations.

“We want to figure out what you guys want to see from your library going forward, not just the next five years, but 10, to 15, to 30,” Library Director Tom Donlon said to residents in attendance during the meeting. “We’ve been doing a lot of research, a lot of work. You guys have had a lot of questions.”

According to the board’s presentation, goals of the eventual renovations will be to relocate the library’s teen center, which is currently across the street from the main building; establish a more functional meeting space than the current one in the library’s basement; provide visitors with access to more computers and other technology; and expand the use of existing space in the main building, among others.

Community members in attendance suggested issues they’d like to see addressed by the project. Some aligned with the board’s plans, including technology expansion and improvements, better use of existing library space and a larger area for group meetings. In addition, residents want to see better elevator access and expansion to connect the property at 205 East Main Street, which the library purchased in 2015.

“It was in good shape, and it lent itself to the history of Port Jeff, and we all know, Port Jeff, we love our history,” Donlon said of the historic house, which was built in 1812, according to the Port Jefferson Historical Society. “We love to honor it, we love to soak ourselves into it, we believe in it. Our best plan of action, not only for the library but for the village, was for restoration and preservation. So that’s the mode we’re getting into.”

The next step in the process will be to formulate a complete design plan to be presented to the community. That meeting is tentatively scheduled to take place in the coming weeks. For more information about the project or to contribute to fundraising efforts, visit www.portjefflibrary.org.

Port Jefferson Village is looking to bring Uptown Funk to Port Jefferson Station, but it’ll need some help.

The Port Jefferson Village board of trustees plans to submit a funding proposal to the Empire State Development Corporation to breathe new life into upper Port Jefferson.

The plans are part of the Restore New York Communities Initiative, which was funded in the 2015-16 state budget for the sole purpose of supporting municipalities in rehabilitating blighted commercial properties.

If awarded, the funding proposal would grant the village up to $500,000 to be used to clean up five adjacent parcels near the intersection of Perry Street and Main Street, about a block north of the Long Island Rail Road station. The village is calling the multiphased project Uptown Funk.

Mayor Margot Garant said during a public hearing on the matter Sept. 26 that the village plans to apply for the grant yearly in the hopes of redeveloping multiple areas in upper Port over time. The grant will also require the village to match at least 10 percent of the $500,000 toward the project, according to Garant.

“The $500,000 can be used for sidewalk restoration, demolition, redevelopment, parking lot improvements — all the things that would be necessary to help a developer make an improvement to this area.”

The location was selected following a blight study in May, which targeted several areas in Port Jefferson Station in need of attention. The buildings named in the funding proposal were ultimately chosen because of the village’s belief that the property owner will cooperate. The grant requires a willing participant from the private sector. Currently the buildings on the property are vacant.

Village grant writer Nicole Christian said she expects to hear back regarding the application by the spring of 2017, and at the moment no concrete parameters have been established for how exactly the money would be put to use.

“The $500,000 can be used for sidewalk restoration, demolition, redevelopment, parking lot improvements — all the things that would be necessary to help a developer make an improvement to this area,” Garant said. “The $500,000 is sort of loosely prescribed, and what I mean by that is we’re not told we have to put it into sidewalks, or told we have to put it into one aspect of the project. So as far as we see it, it enables the village to bring $500,000 to the table to help incentivize a project that will give back to the village perhaps more of what it would like to see, which is a strong, anchor retail establishment on the main floor, or a restaurant with housing above.”

Trustee Bruce D’Abramo expressed his excitement to get the project started.

“I’m really happy to see the village moving forward on this particular issue,” he said of the revitalization of Port Jefferson Station. “It has been a clear goal of mine since I became a trustee to do something about upper Port, and this is one of the mechanisms that I’m happy we can embrace.”

Trustee Larry Lapointe also voiced support for the plan.

“I think this particular corner is perhaps the worst corner uptown,” he said. “The two buildings that are on site were deemed to be so unsafe that we had to vacate and board them up. Two of the lots behind are magnets for homeless people, and we’re constantly working with the owners to get camps moved out of that area when they spring up. It’s sorely in need of redevelopment.”

Barbara Ransome, director of operations for The Greater Port Jefferson Chamber of Commerce, addressed the proposal during the hearing.

“The Greater Port Jefferson Chamber of Commerce certainly supports this potential funding and really feels it’s very important, especially in upper Port, in our business community there, and as a gateway coming into the village,” she said. “It’s critical for this type of development to continue.”
